[NARRATIVE #1 on March 02, 2026]
On Wednesday, March 2nd, [2026], at approximately 2202 hours, I, Ofc. Perry #149 responded with Winder Officers to [100 BLOCK] Glenview Terrace, Winder Ga 30680, in reference to a suspicious person. I traveled to the location listed above, CODE 1, in my marked patrol vehicle #400.
Upon arrival, Ofc. Dutcher #141 and Ofc. Harris #121 had already detained Mr. Eight Slayton. I arrived on scene as backup for them. As Eight was being patted down, he started to become agitated and found it best for him to be placed in the backseat of Ofc. Harris #121 marked patrol vehicle #523. Eight also has warrants out of the county that we were waiting to be confirmed.
As we waited for confirmation of the warrants, Eight became irate and acted irrationally. Eight began hitting his head on the clear divider between the backseat and the front seat. I told Eight to stop hitting his head on the glass, but he continued. I determined that, for the safety of Eight and others, it was best to place him in the WRAP to prevent any injury to himself.
I called Cpl. Morris #145 to the scene with the WRAP, while we were waiting for the WRAP to arrive, Eights emotions and attitude continued to escalate. When Cpl. Morris #145 arrived on scene; we removed Eight from the back seat of Ofc. Harris #121 patrol vehicle and began placing him in the WRAP. I assisted with securing the ankle straps and putting the leg buckles in place.
Eight was then placed back inside of Ofc. Harris #121 patrol vehicle and a lap-and-shoulder belt were secured across his person. I then followed Ofc. Harris #121 to [REDACTED BY AGENCY] for a [REDACTED BY AGENCY] for Eight. Eight was given [REDACTED BY AGENCY] at the hospital, and I returned to service with no further issues.

[NARRATIVE #2 on March 02, 2026]
On March 4, 2026, at approximately 2202 hours, I, Officer Jacob Harris, was dispatched to [100 BLOCK] Glenview Ter, Winder, Ga, 30680 for a suspicious person. Dispatch advised that an unidentified black male wearing an orange shirt and blue jeans was on the caller’s porch. I responded CODE 1(no lights and siren) in my marked patrol unit #523 to the location.
Upon arrival, I observed a black male who was identified as Eight Slayton, who matched the description given by dispatch on the porch of [100 BLOCK] Glenview Ter, unscrewing a light bulb. I advised Mr. Slayton to stop what he was doing and to come off the porch. Mr. Slayton complied with my commands. I have a prior history with Mr. Slayton and knew he had a warrant from the City of Winder. I advised Dispatch that I was out with Mr. Slayton. I advised Mr. Slayton that he was being detained. I asked Mr. Slayton to turn around and place his hands behind his back. Mr. Slayton complied with my commands. I placed Mr. Slayton’s wrists in handcuffs, double-locked them, and checked them for proper fit behind the back. I walked Mr. Slayton to my marked patrol vehicle, where I patted him down for any weapons. Mr. Slayton didn’t have any weapons on his person.
Cpl. Morris advised dispatch that Mr. Slayton had a warrant out of the City of Winder and was en route to conform it.
I advised Officer Perry and Officer Dutcher that they could stay with Mr. Slayton so I could talk to the caller. I approached the caller, identified as [VICTIM #1], who was sitting in her car. I asked Ms. [VICTIM #1] what had happened. Ms. [VICTIM #1] stated that she was about to leave to take her mother home when she saw Mr. Slayton walk from across the street to her property and went onto her porch and started unscrewing her light bulb and busted it on the porch. Ms. [VICTIM #1] stated that he started banging on the walls, picking up her trash can, and throwing it off the porch toward them. Ms. [VICTIM #1] stated that he went to the neighbors next door and started to unscrew their door while she was on the phone with dispatch. Ms. [VICTIM #1] stated that this is when we show up at her residence. Officer Dutcher took photos of the damaged light bulbs, which will be attached to this report. Officer Dutcher gave Ms. [VICTIM #1] a case card with a case number on it and advised her on how to obtain a copy of the report.
Cpl. Morris advised that the warrant was valid and placed a hold. Officer Perry searched Mr. Slayton incident to arrest for weapons, contraband, and any means of escape. Mr. Slayton didn’t have any contraband on his person. Officer Perry and Officer Dutcher walked Mr. Slayton to the left of my marked patrol vehicle. I checked my back seat for contraband before placing Mr. Slayton inside and properly securing him with a seatbelt. I placed Mr. Slayton’s belongings in the back of my patrol car.
Mr. Slayton started banging his head against the intercage. Officer Perry W149 advised Mr. Slayton to stop multiple times. Mr. Slayton continued to bang his head on the cage. Officer Perry advised Cpl. Morris to bring the WRAP Restraint to our location. Mr. Slayton was going to be checked out by EMS, but due to him acting in a manic state and talking out of his head. We placed Mr. Slayton in the WRAP for his well-being and the safety of others. I transported Mr. Slayton to the [REDACTED BY AGENCY] in my patrol vehicle #523 for [REDACTED BY AGENCY].
Mr. Slayton was seen by medical personnel [REDACTED BY AGENCY] I released Mr. Slayton to the hospital [REDACTED BY AGENCY] and a hold was placed on Mr. Slayton.
I will be procuring a warrant for Mr. Slayton for Disorderly Conduct.

[NARRATIVE #3 on March 04, 2026]
On 03/04/2026 at approximately 2203 hours, Ofc. J. Harris W121 and Ofc. J. Dutcher W141 responded to a suspicious person call at [100 BLOCK] Glenview Terrace, Winder, Ga, 30680.
Upon their arrival, they located the suspect, identified as Mr. Eight Slayton. Upon hearing their radio traffic, I, Cpl. T. Morris W145 advised the officers on the scene that Eight Slayton had active arrest warrants. Ofc. Harris then detained Mr. Slayton. Once the warrants for Eight Slayton were confirmed, Ofc. Harris notified Barrow County Dispatch that the subject was under arrest.
A short time later, Ofc. J. Perry W149 advised me via radio that he needed the WRAP restraint device because Eight Slayton had begun hitting his head on the partition in the patrol vehicle and continued this behavior after being instructed to stop several times. I then advised dispatch that I was Code 3 (emergency equipment activated) to the P.D. to retrieve the WRAP device. Upon retrieving the device from the P.D. I traveled Code 3 to the scene where the device was laid out in a grassy area.
Eight Slayton was then assisted from the back of Ofc. Harris patrol vehicle was slowly assisted to the ground on his stomach by Ofc. Perry, Ofc, Harris, Ofc. Dutcher, Ofc. Simmons W164 to prevent further harm. Upon having Eight lie down, I assisted Ofc. Simmons by holding Eight Slayton’s ankles together while Ofc. Simmons applied the ankle restraint. I then assisted Ofc. Perry and Ofc. Harris by securing the seatbelt couplings in the knee area. Ofc. Perry and Ofc. Harris checked the couplings for proper fit. At the same time, Ofc. Dutcher placed the chest/back portion of the Wrap device on Eight Slayton.
Once all of the lower half of the WRAP Device was secured on Eight Slayton. I gave the command for everyone to roll Eight Slayton to his left onto his back. This was completed by all officers in a slow and secure manner. Once Eight Slayton was on his back, Ofc. Harris, Ofc. Dutcher and Ofc. Perry sat Eight Slayton up in a sitting position. The handcuffs that were securing his hands behind Eight Slayton’s back were then secured into the WRAP device by Ofc. Harris. Ofc. Dutcher and I secure the side buckles for the upper portion of the device. I then secured the front strap/buckle, preventing Eight Slayton from falling backwards.
Eight Slayton was then lifted using the WRAP device handholds by Ofc. Perry, Ofc. Simmons, Ofc. Dutcher and Cpl. Morris. Eight Slayton was placed into the passenger compartment of marked patrol vehicle #523. Where Eight Slayton was secured using the lap and shoulder belt. Ofc. Harris then transported Eight Slayton to [REDACTED BY AGENCY] for [REDACTED BY AGENCY].
Eight Slayton was placed in the WRAP restraint for his safety and well-being to prevent further self-harm and protect others around him.

[NARRATIVE #4 on March 15, 2026]
On March 15, 2026, at approximately 2311 hours, I, Officer Jacob Harris, was dispatched to a dispute at [100 BLOCK] James Albert Johnson Ave, Winder, GA, 30680. I responded CODE 1 (no lights and sirens) in my marked patrol unit #523 to the location.
Upon arrival, I met the caller, Eight Slayton. I asked Mr. Slayton what had happened. Mr. Slayton, someone took his debit card and threw it away. Cpl. Morris asked Mr. Slayton whether he wanted a report for his debit card. Mr. Slayton advised that he didn’t want a report and didn’t know why he called.
Officer Dutcher advised dispatch of Mr. Slayton’s information. Dispatch advised that Mr. Slayton had a warrant out of Barrow County and the Winder Police Department. A short time later, Dispatch advised that Mr. Slayton’s warrant was valid.
Mr. Slayton was handcuffed behind the back, double-locked them, and checked them for proper fit. I advised Mr. Slayton that he was being arrested on two warrants out of the Winder Police Department. I walked Mr. Slayton to my marked patrol vehicle, and I searched him incident to arrest for weapons, contraband, and any means of escape. Mr. Slayton didn’t have any contraband on his person. I walked Mr. Slayton to the left of my marked patrol vehicle. I checked my back seat for contraband before placing Mr. Slayton inside and properly securing him with a seatbelt. Mr. Slayton’s belongings were given to his mom at Mr. Slayton’s request.
I left the scene en route to the Barrow County Detention Center. I released Mr. Slayton and his belongings to the staff without incident. After transport, I checked the rear of my patrol vehicle for contraband, but no contraband was found.
