[NARRATIVE #1]
On January 9, 2022, at approximately 0139 hours, I responded lights and sirens, “code 3” to Chips Bar and Grill for a Fight. While en route, dispatch stated that the caller, one of the bartenders stated that there was a subject involved in the fight with a gun. Dispatch also updated the responding units and stated that there was approximately 8-12 people fighting. I asked dispatch to stop all radio traffic on our channel because I was almost on scene. As I was approaching the parking lot, a dark in color SUV left the scene. I parked my patrol vehicle in front of one of the exits stopping additional cars from leaving the scene.
I exited my patrol vehicle and drew my pistol from its holster. I could see at least two people in a white SUV in front of my patrol vehicle. I gave verbal commands for the occupants in the car to show me their hands. Two males, later identified as Matthew Mainard and Zeyad Nassar, began slowly walking towards the white SUV. I then gave Matthew and Zeyad loud verbal commands for them to show me their hands and to stop moving. Matthew and Zeyad continued slowly walking towards the vehicle. I again gave the loud verbal commands for the two to stop moving. Matthew and Zeyad at this point had their hands in the air. Matthew was yelling things I do not remember and cussing. I told Matthew to relax.
Deputy Stephens arrived on scene and began giving loud verbal commands for everyone to get on the ground. Matthew and Zeyad were standing beside the white SUV and had to be told numerous times to get on the ground and were even told that if they did not comply that the dog would be released. Eventually, Matthew and Zeyad got partially on the ground. At this point, everyone I could see was lying on the ground. Deputy Stephens and I slowly advanced towards Matthew and Zeyad while attempting to watch the others at the other end of the parking lot. Lieutenant Hansen arrived on scene and approached Deputy Stephens and I to assist with detaining Matthew and Zeyad. Matthew and Zeyad continued to yell, and Lieutenant Hansen told Matthew to place his hands behind his back. Matthew did not immediately comply. Lieutenant Hansen pulled his taser and told Matthew if he did not place his hands behind his back, he would be tased. Upon hearing this and seeing the taser, Matthew placed his hands behind his back. Lieutenant Hansen detained Matthew.
Once Matthew and Zeyad were detained, I told Deputy Stephens that the white pickup truck on the other side of the white SUV we were standing beside had occupants in it. Deputy Stephens and I began walking towards the white pickup truck. Deputy Stephens gave both occupants loud verbal commands to put their hands up and exit the vehicle. Both occupants did as instructed and the male was identified as Chadwick Snyder. Deputy Stephens then assisted other deputies on scene secure others at the other end of the parking lot. While I was standing at an angle where I could see Matthew, Zeyad and Chadwick, Zeyad began to sit up right and asked if he could stand up. I told him no and he asked why. I told Zeyad because I said no. At this time, Deputy Stephens was walking back towards my side of the parking lot and told Zeyad to lie back down. Deputy Thurman and I approached Zeyad and told him to lie back down on the ground. Zeyad refused and Deputy Thurman and I placed Zeyad back on the ground. I did this by pushing Zeyad’s back towards the ground. Zeyad did not want to move his legs out from under him, so I pulled his right leg out from under him so that he was flat on the ground.
Zeyad was saying that he was going to sue the county amongst other things and causing a scene. Lieutenant Hansen stated that since Zeyad was lying on the ground to search him because he was going to jail for obstruction and so was Matthew. I searched Zeyad incident to arrest for contraband and none was located. Deputy Thurman and I escorted Zeyad to my patrol vehicle. I opened the door and instructed Zeyad to have a seat. Zeyad sat in the back of my patrol vehicle but would not put his legs in the back of the car. Zeyad was instructed to put his legs in the patrol vehicle numerous times but refused to do so. Deputy Stephens pushed Zeyad into the back of my patrol vehicle, while I was on the opposite side of the car. I grabbed the back of Zeyad’s jackets and pulled him to the opposite side of the vehicle, so his body was completely in the back seat and the doors were shut.
At this point I heard someone on scene state that the owner of the bar, [VICTIM #1] wanted everyone on scene, except for a select few to be served with Notice of Prohibited Entries. I went back to the white pickup truck and gathered Chadwick and his girlfriend’s information. Chadwick stated that he believed he had a warrant out for his arrest in Coweta County but was told that it was recalled. I provided dispatch with his information. Dispatch confirmed that Chadwick had an active warrant and Coweta wanted to place a hold on him.
I then asked Deputies Thurman, Surine, and Wyatt to assist me in getting the Notice of Prohibited Entries signed. I went inside the bar and spoke with [VICTIM #1], and the two bartenders, [WITNESS #1], and [WITNESS #2]. I was told that [WITNESS #2] was the one that called 911. Lieutenant Hansen was also speaking with [WITNESS #2] and [WITNESS #1]. I asked [WITNESS #1] to explain to me what happened since [WITNESS #2] stated that she did not leave from behind the bar. [WITNESS #1] stated that she went outside because she saw two women arguing back and forth and it was getting heated. [WITNESS #1] stated that once outside, she saw the two-woman going at it and being separated. [WITNESS #1] stated that during this, she heard one of the males tell another to not put his hands on a woman which started a fight between four males. [WITNESS #1] stated that she saw a gun in a male wearing a camo jackets waste line, later identified as Christopher Plemmons and she tackled him to break up the fight.
Based off [WITNESS #1]’s description of the males which was the big guy with the red beard, Matthew and his skinny friend, Zeyad, the guy in the camo jacket, Christopher and the guy with long hair, Chadwick were identified as the four males fighting. I then went outside and over to Chadwick. I informed him that he had a valid warrant out for his arrest in Coweta County and they wanted place a hold. I also informed him based of witness testimony, he was involved in the fight and would be going to jail for Affray. I instructed Chadwick to turn around and place his hands behind his back. Chadwick did as instructed and I placed him in a pair of double locked handcuffs behind his back and checked them for proper fit. Sergeant Jordan escorted Chadwick to the back of a patrol vehicle. Someone on scene also informed Christopher that he was under arrest for Affray and placed him in the back of a patrol vehicle.
I went back inside and asked [WITNESS #1] if she would be willing to fill out a witness statement and she was hesitant and asked if she had too. I informed her it was voluntary, and she stated that she did not want to in case one of the males wanted to retaliate. [VICTIM #1] asked that if vehicles were left in her parking lot over night due to the drivers intoxicated states that they be removed the next day before 1230 hours.
While informing a driver that her vehicle needed to be picked up by 1230 hours this day, Amanda Walden approached Lieutenant Hansen, Deputy Stephens and I. Amanda stated that the whole reason the men we had in custody got into a fight was because Matthew put his hands on her. Amanda stated that while she and another female were arguing, Matthew came up to her and shoved her. Amanda stated that Chadwick and Christopher told Matthew not to put his hands on a woman and that was what started the fight. Lieutenant Hansen asked Amanda if she wanted to pursue charges. Amanda was hesitant and asked if she did if that would stop some of the males from going to jail and we told her that it would not. Lieutenant Hansen told Amanda that if she did want to pursue charges that she could go and speak with a Magistrate judge and take her own warrants. I gave Amanda a case card with my name, badge number and the case number on it. I then transported Zeyad to the Barrow County Detention Center and turned him over to the staff. The back of my patrol vehicle was searched before and after transport for contraband and none was located.
Prior to transporting Zeyad to the detention center, Deputy Wyatt told me that he filled out a Notice of Prohibited Entry for Zeyad, but he did not have him sign it due to him being in handcuffs. Once at the jail, I explained the Notice of Prohibited Entry to Zeyad and had him sign it once he was in a holding cell. A picture of the notice was taken and attached to this report.
I applied for and Affray warrant for Matthew, Zeyad, Chadwick, and Christopher. I also applied for a misdemeanor obstruction warrant against Matthew and Zeyad.
No warrants were taken for Simple Battery against Matthew for his alleged shove of Amanda due to it being a misdemeanor that happened outside my presence.

[NARRATIVE #2]
On January 9, 2022 at approximately 0138 hours I was made aware of a fight located at 655 Patrick Mill Road, Chips Bar and Grill. Due to the call notes about firearms being involved and a large number of people fighting I responded “code3.”
Just prior to my arrival I was made aware that Deputy Johnson arrived on scene and advised that he had multiple people at gun point. Arriving a few moments later I observed that no less than eight people where refusing to comply with Deputy Johnsons’ commands. I exited my vehicle and unholstered my duty weapon and begin giving loud verbal orders for the loudest male to lay flat on the ground, still refusing all lawful orders given to him I removed K9 Dax, due to Deputy Johnson and myself being out numbered five to one, and connected K9 Dax to a tactical lead and started pushing forward towards the first group of offenders.
Giving loud verbal orders for multiple people to comply under the threat of being dog bit or tased I used K9 Dax to clear multiple vehicles of occupants, placing them prone on the ground. While other units arrived on scene to assist. Once all available units arrived I placed K9 Dax back in the vehicle and began to oversee the identification of all subjects involved. I remained on scene until all offenders had been arrested and all others had been identified and issued notices of prohibited entrees.

[NARRATIVE #3]
On 09 January 2022 at approximately 0138 hours, I heard several units dispatched to 655 Patrick Mill Road SW (Chips Bar & Grill) in reference to a fight. Per the caller, the fight was between females and males totaling about (6-8) six to eight people. Units began responding lights and siren (code 3). Updates continued to be given to responding units. These indicated the fight had increased in numbers and that a subject in a camouflage jacket had a firearm.
Upon the first unit’s arrival the channel was cleared for emergency traffic only. I heard Dep. Johnson advise Dispatch he had one at gun point. Dep. Stephens arrived next. I was the third unit to arrive on scene.
Arriving on scene at 655 Patrick Mill Road SW, I entered the parking lot utilizing the first entrance as the second entrance, closest to Roxey Maxy Road was partially blocked by patrol vehicles. Pulling in I observed approximately (20) twenty plus people spread out throughout the parking lot. Most were lying on the ground while several were still milling around their vehicles. Exiting my patrol vehicle, I observed Dep. Stephens and Dep. Johnson addressing multiple people, giving loud verbal commands for them to stay down and stop talking. Dep. Stephens had his K9 partner Dax out as a show of force/force option if needed. Of the approximately (10) ten people I quickly observed, I did not see a male in a camouflage jacket. This gave me concern as he was reportedly the one with the firearm.
I did observe a white male, later identified as Matthew Mainard to be lying on the ground but pointing his hand towards several other people. Matthew was still being belligerent, boisterous, and disorderly. He was cussing and shouting and causing Deputies to have to focus more attention on him while still attempting to gain control of the situation. I immediately pulled my TASER Conducted Electrical Weapon, activating it and painting Matthew with the laser. Matthew was ordered to place his hands behind his back, or he would be tased. Mattew complied and was secured into two sets of handcuffs.
Beside Matthew was apparently a friend of his, later identified as Zeyad Nassar. While I was handcuffing Matthew, I observed Zeyad on no less than two separate occasions roll on his side or attempt to move closer. Zeyad was told multiple times to remain facedown on the ground. I observed Zeyad to have a small abrasion to his forehead. When asked Zeyad declined medical attention. Even handcuffed, Matthew was still being so belligerent, boisterous, and noncompliant he was garnering the attention of at least two deputies while there were still multiple people inside and around vehicle’s that had yet to be secured or searched. Zeyad was also still moving around and not cooperating.
As more units arrived law enforcement was finally able to get the parking lot calmed down. A male with a camouflage jacket was observed and ordered to the ground and handcuffed. At least (3) three firearms were found on scene, see responding deputies’ supplements for their locations and names of subjects who possessed them.
I entered the establishment and contacted the on-duty bartenders. They were identified as [WITNESS #2], the caller and witness [WITNESS #1]. I asked them to explain what happened. [WITNESS #1] advised the incident began out back on the porch. She stated initially it appeared to be between two females, however she stated she was unable to identify them other than as a blonde and a brunette. [WITNESS #1] continued by telling me when she went out the incident turned from the two females, into involving four males. When asked if she could identify the males, [WITNESS #1] told me, the bigger guy with a red beard (Matthew Mainard), his dark complexion friend (Zeyad Nassar), a guy wearing a camouflage jacket, who she tackled because she observed a gun on his side (Christopher Plemmons), and a skinny male with shoulder length brown hair (Chadwick Snyder).
[WITNESS #2] stated when [WITNESS #1] went outside, she had stayed inside the bar and updated Dispatch as to what was going on. [WITNESS #2] stated at some point the altercation came into the bar then returned outside where everyone began running towards the front of the establishment.
[VICTIM #1] then introduced herself. [VICTIM #1] advised she and her daughter were the owners of Chips Bar & Grill. [VICTIM #1] continued by explaining she lived next door and had heard the commotion at the establishment. She continued by stating she had come over to see what was going on and witnessed everyone running towards the front. [VICTIM #1] stated the only reason she could think that people would run the way they were was a gun was involved. [VICTIM #1] stated she gathered [WITNESS #1], [WITNESS #2] and herself and they all locked themselves inside the establishment until law enforcement arrived. As [VICTIM #1] is one of the owners of the bar and grill, she notified law enforcement she wanted numerous people involved banned or served with prohibited entry notices.
Dep. Johnson came inside, and I explained what I had learned from [WITNESS #1] and [WITNESS #2] as well as relaying [VICTIM #1]’s request. Dep. Johnson then spoke with [WITNESS #1], [WITNESS #2], and [VICTIM #1]. I went outside and began instructing other deputies on gathering information and beginning the process of paperwork for notices of prohibited entry.
Due to Zeyad’s and Matthew’s belligerent, boisterous, and non-compliance with orders given during the initial arrival of law enforcement, while attempting to gain control of the tense, uncertain and rapidly evolving incident, they were arrested for obstruction (misd.). At the conclusion of law enforcements investigation, Matthew Mainard, Zeyad Nassar, Christopher Plemmons, and Chadwick Snyder were all identified as having been involved in some type of physical altercation. As this was the case, each male was also charged with affray for participating in mutual combat in a public place.
Several vehicles had to be left on the premises due to subjects being intoxicated and unable to drive. [VICTIM #1] provided a 1230 pm time for these vehicles to be removed or they would be towed. Amanda Walden came up prior to us leaving and informed us that a bigger red bearded male (Matthew Mainard) and shoved her on the back porch/patio and this was when several male friends of hers had stepped in and the males got involved in a physical altercation. Amanda wanted this documented and went so far as to ask if she pressed charges for the simple battery would it change the outcome of the subjects (her male friends) that had been arrested. Amanda was informed that it would not change the outcome for the night. Amanda was asked multiple times if she wanted a report filed regarding simple battery, however continually changed her mind. Ultimately, she was provided a case card and explained the process for a Magistrate Court warrant hearing.

[NARRATIVE #4]
On Sunday, January 9, 2022, at approximately 0146 hours, I arrived on scene to a fight at Chips Bar and Grill at 655 Patrick Mill Road. I spoke with the following people and obtained their information: Perry Avilla, Amanda Walden, Valarie Turk, Jessica Daniel, Elizabeth Avilla, and Jessica Lumkin. I met with Ms. [VICTIM #1], owner of Chips, and asked how long she’d like the band on the Notice of Prohibited Entry form to be. Ms. [VICTIM #1] stated she wanted a lifetime band. I filled out the forms, had Ms. [VICTIM #1] sign all the forms, and Ms. [WITNESS #1] sign as a witness to all the forms.
The following people signed the Notice of Prohibited Entry, and they were given a pink copy of Notice of Prohibited Entry – Lifetime: Perry Avilla (lifetime), Amanda Walden (lifetime), Valarie Turk (lifetime), Jessica Daniel (lifetime), Elizabeth Avilla (lifetime), and Jessica Lumkin (lifetime). Each person was informed if they returned to Chips Bar and Grill, they would be arrested for Criminal Trespass.
[NARRATIVE #5]
On 01/09/2022 I responded code 3 (lights and siren) to a fight in progress with multiple subjects involved at Chips Bar and Grill located at 655 Patrick Mill Rd SW. While enroute it was reported that one person had a gun and was brandishing it. I heard Deputy Johnson come across the radio and say he had the crowd at gun point.
I arrived on scene and there were multiple deputies already on scene. I observed Lt. Hansen hold a male at Taser point. He was giving the male verbal commands to the male to put his hands behind his back and that he was under arrest. I ran over to Lt. Hansen and put my knee on the males buttocks to restrict his freedom of movement to gain control of him. I gave the male verbal commands to put his hands behind his back. I had to wrestle his right arm behind his back. The male kept pulling his left arm away form me. I told him to stop. I was eventually able to place the male in handcuffs behind his back. Due to the rapidly evolving situation on scene I didn’t double lock the cuffs or check for tightness. There were a total of 25 people involved in the fight.
I then went to Deputy Stephens who had K9 Dax Deployed. Deputy Stephens and multiple other deputies had another male and female at gun point. It was reported that the male they had at gun point had firearms with him. Once it was safe to do so I moved in with Deputy Wyatt to detain the male and female. The male was identified as Mr. Christopher Plemmons. I handcuffed Mr. Plemmons behind his back. I didn’t double lock them or check for tightness due to the exigent circumstances that were displayed on scene. Once the scene was secured and contained. I assisted Deputy Wyatt. Deputy Wyatt was searching Mr. Plemmons vehicle and recovered 3 firearms. 2 Taurus G2 9mm’s, and an American Arms Deringer .22lr. I ran the serial numbers on the firearms through dispatch and they came back with no wants on them.
I went to Deputy Stephens, and he directed me to start issuing notices of prohibited entry. Everyone involved in the fight was getting banned from Chips for a lifetime. I served Mr. Richard Turk, and Mr. Plemmons with Notices of prohibited entry. I also assisted in identifying Mr. Turk, Mr. Perry Auville, and Ms. Rebecca Auville.
Mr. Plemmons was arrested for Affray. Mr. Plemmons was placed in the right rear of my patrol car. Other deputies performed a search of Mr. Plemmons. I transported Mr. Plemmons to the Barrow County Detention Center without incident. Once we arrived at the detention center, I transferred custody over to detention deputies.
This concludes my involvement in this case.

[NARRATIVE #6]
On, 01/09/2022, at approximately 0138 hours deputies responded code three (emergency lights and siren activated) to Chip’s Bar and Grill located at 655 Patrick Mill Road in reference to a fight. The outcome of this fight resulted in multiple people being arrested.
While en route dispatch advised deputies a male wearing a camouflage jacket and camouflage hat had a gun during the fight. A brief period of time after being informed of this information the first deputies arrived on scene. I heard one of the deputies say on the radio they had an individual at gunpoint. I continued my emergency response to the scene to assist the first responding deputies.
Upon my arrival I observed multiple individuals laying on the ground who needed to be detained to determine who was involved in the fight. I approached a black car that was parked in the parking lot with multiple individuals laying on their stomachs around the car. At this vehicle I detained two individuals in handcuffs behind their backs. These two individuals were, later identified as Valerie Turk and William Harder.
After detaining William and Valerie I went across the parking lot to a grassy area where a dark colored Ford pickup truck was parked with a male and female next to it. When I got to the truck, I detained a female, later identified as Heidi Davis, in handcuffs with her hands in front of her body. I handcuffed Heidi in front of her body due to her having a laceration on two of her fingers as a result of the fight.
As I was detaining Heidi Deputy Surine (S-150) detained a male who was laying on his stomach beside Heidi. The male was, was later identified as Christopher Plemmons. When Chirstopher was detained in handcuffs, I noticed he was wearing a camouflage hoodie matching the description of the male who had a gun given by dispatch prior to my arrival. When Heidi and Christopher were detained and secured, I began to assist Heidi with her lacerated hand. I ultimately had to go to my patrol vehicle and get gauze for Heidi to apply to her wound.
As I was beginning to provide medical aid to Heidi Deputy Stephens (K9-1) notified me via radio that Christopher had a gun. I then conducted a Terry frisk of Christopher’s person for the weapon. When I began patting Christopher down, I observed a knife clipped to his front right pants pocket and removed it. Christopher informed me there was an additional knife in the same pocket I removed the first knife from. I removed that knife as well and continued my frisk of Chirstopher’s person. I did not find any other weapons on him but did observe an empty black handgun holster on the right side of his belt.
I asked Christopher where the gun was, and he stated it was in the center console of his truck. Christopher also informed me there was a firearm in Heidi’s purse which was also in the truck and a third firearm in the glove box of the truck. I located all three firearms in the locations Christopher stated while he was present. The firearms were as follows (pictures of all firearms attached to report):
A tan and black Taurus 9mm semi-automatic handgun (serial number: TLY90302).
A mint green and silver colored 9mm Taurus semi-automatic handgun (serial number: TLW98816)
A silver-colored American Arms .22 caliber revolver styled handgun (serial number: L240241).
All firearms located were run through NCIC/GCIC and it was determined they were not stolen. At the conclusion of the on-scene matters handled by law enforcement the firearms were returned to Heidi.
Briefly after my arrival EMS personnel arrived on scene to evaluated individuals on scene involved in the fight. I walked with EMS personnel as they evaluated each individual who needed to be seen by them. No one was transported by EMS at the time of the incident.
After all individuals were cleared by Ems, EMS personnel left the scene.
Deputies then began to issue criminal trespass forms to individuals who were involved in the fight. I filled out a criminal trespass form for an individual who was, identified as Zeyad Nassar. Zeyad did not sign his criminal trespass form while on scene due to him already being in handcuffs and in the back of a patrol vehicle. Zeyad later signed his criminal trespass from after being transported to the jail by Deputy Johnson (S-198).
An individual who was, identified as Chadwick Snyder had been placed in the rear passenger seat of my patrol vehicle during the incident. When I was no longer needed on scene, I transported Chadwick to the jail where he was turned over to the jail staff without incident and went back in service.

[NARRATIVE #7]
On January 09, 2022, at 0138 hours, I responded Code 3 (emergency lights and siren) to 655 Patrick Mill Rd (Chips Bar and Grill) for a fight in progress.
Upon arrival, I assisted in securing the scene with the other deputies that had arrived before me. After the incident scene had been secured, the owner of Chips Bar and Grill made the decision to serve a Notice of Prohibited Entry (NPE) on multiple people that were involved in the fight.
I identified Ms. Heidi Davis, ran her information through GCIC, and added her to the call. I then filled out a NPE for her. I explained that the NPE was for a lifetime ban and that if she returns to Chips, she is subject to arrest for Criminal Trespass. Both the owner and Ms. Davis signed the NPE and each were given their copies.
