Inspector Notes: Observed food service employee handle shredded cheese with bare hands. Shredded cheese is RTE. CA: Unless the conditions specified in DPH Rule 511-6-1-.04(4)(a)4. are met, employees are preventing cross-contamination of ready-to-eat food with bare hands by properly using suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment. COS: PIC discarded container of shredded cheese.

Inspector Notes: Observed main cook exit main kitchen and re-enter and begin handling food with same gloves, not changing gloves or washing hands. CA: If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. COS: PIC instructed employee to remove gloves, wash hands, and don new gloves.

Inspector Notes: Observed all potentially hazardous TCS food items in properly cold holding above 41F in the walk-in cooler. See temp log. Internal air thermometer was reading 50F upon entering the cooler. Observed sauce bottles in RIC in waitress station improperly cold holding above 41 degrees F. CA: Cold held TCS food shall maintain an internal temperature of at least 41F or below. COS: PIC unsure of how long WIC had been out of temp and all food was discarded. Sauces in RIC are pulled in and out throughout day, unsure of how long they were out of temp. Sauces were discarded.

6-1D – time as a public health control: procedures and records

Inspector Notes: Observed no discard or start time, or temperature verification available at time of inspection. WIC where food is being pulled is temping at 46 degrees F. CA: If time without temperature control is used as a public health control for TCS foods, the food shall have an initial temperature of at least 41F when removed from temperature control and the food shall be discarded at 4 hours. COS: PIC discarded food.

Inspector Notes: Observed several sauce bottles throughout facility without any labeling indicating its contents. CA:Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.

Inspector Notes: Observed floors, walls and ceilings throughout the facility in need of deep cleaning and proper repair. Observed large amount of exposed wood in dining area and exposed wood in restroom. CA: Physical facilities shall be maintained and all floors/walls/ceilings shall be smooth and cleanable.

Inspector Notes: Observed employee personal drinks stored over facility food in waitress station RIC. CA: Designated areas for employee personal items shall be used to prevent the contamination of food, equipment and utensils. COS: All items were relocated.
