On 19 March 2026 at approximately 20:37, I was dispatched to [10 BLOCK] Mongomery Street apartment [REDACTED] in reference to a physical domestic. Dispatch advised that while en route, the (complainant), [VICTIM #1], got hit over his head with a glass bottle and is bleeding.
I arrived, and I observed Mr. [VICTIM #1] walking away from [10 BLOCK] Montgomery Street Apt [REDACTED] on Tallapoosa Street. I stopped and asked him what happened. He reported that he got hit over the head with a glass bottle and is bleeding. I requested Mr. [VICTIM #1] to walk back over to [10 BLOCK] Montgomery Street.
I arrived, and I met with (offender) Rachel Phillips outside in the front yard. She stated that she and Mr. [VICTIM #1] got into an argument, and she told him to leave. I asked Mrs, Phillips if she hit Mr. [VICTIM #1] over the head with a glass bottle, and she stated “yes”.
Once Mr. [VICTIM #1] came back to [10 BLOCK] Montgomery Street, I asked him if he needed an ambulance, and he stated “yes”. Mr. [VICTIM #1] showed me the glass bottle that he was hit over the head with, and I took digital pictures with my department-issued cellphone of the glass bottle, the bleeding injury on the back of his head, his front face, and the top and bottom of his hands.
I placed Mrs. Phillips under arrest for Aggravated Assault Family Violence O.C.G.A 16-5-21, I placed handcuffs to her back, checked for fit, and double locked. I placed Mrs. Phillips in the back seat of Officer Curtis’s patrol car. I did not see any visible marks or injuries on Mrs. Phillips.
I requested two (witnesses), [WITNESS #1] and [WITNESS #2], to complete witness/victim statements to be picked up at a later time.
Tanner West Georgia ambulance attended to Mr. [VICTIM #1], and he refused transport to the emergency room. I requested Mr. [VICTIM #1] to complete witness/victim statements to be picked up at a later time. Mr. [VICTIM #1] stated he did not want Mrs. Phillips to be arrested. I advised Mr. [VICTIM #1] that, under Family Violence, if I could identify the primary aggressor, I would have to make an arrest. I advised Mr. [VICTIM #1] that Mrs. Phillips is being charged with Aggravated Assault Family Violence because of the injury he sustained on the back of his head.
I gave Mr. [VICTIM #1] a Domestic Violence family crisis, victim, and witness assistance information pamphlet. I also explained how to file for a temporary restraining order through the Carroll County women’s shelter.
Officer Curtis transported Mrs. Phillips to the Carroll County Jail.
