[NARRATIVE #1 on February 17, 2026]

I responded to an accident in which the vehicle struck the guardrail. The driver had left the scene of the accident and left the vehicle. I began looking for someone walking on the highway near the accident location. I observed a Hispanic male walking on I-95 southbound around the 103-mile marker. I held out with the male, and due to a language barrier, APO Samaniego assisted me in translating. While I was waiting for APO Samaniego, I could smell an odor of alcohol coming from the male, and his shoes were covered in mud.
APO Samaniego stated that the male told him he had been in an accident and that the other passenger had left the scene. The male was placed in double-lock handcuffs. Upon searching his person, I found a key for an older Toyota in his left pocket. The male stated his name was Junior Omar Flores-Ulloa.
Officer Smith searched the vehicle and found two cellphones. Mr. Flores-Ulloa stated that his phone had a blue case, which was found in the driver’s seat. There was paperwork in the vehicle, which all belonged to Mr. Flores-Ulloa. Officer Smith stated that the back seat of the vehicle had items in both seats, and he did not believe there was room for a third passenger.
Mr. Flores-Ulloa had given two different stories to police officers. When APO Samaniego spoke to him, he stated there were two people in the vehicle during the accident and that the other ran to the trees on the side of the highway. When I brought Mr. Flores-Ulloa to the EMS team at the accident scene, he told them there were three people in the vehicle. Mr. Flores-Ulloa refused care from EMS.
Mr. Flores-Ulloa was placed in double locked handcuffs, searched, and seat belted in the back of my patrol car.
Mr. Flores-Ulloa was read his implied consent for 21 and older, and he agreed to the state’s blood test. Mr. Flores-Ulloa was transported to the Pooler Police Dept for a blood test. Unfortunately, a blood draw could not be collected due to Mr. Flores-Ulloa’s dehydration.
Mr. Flores-Ulloa was transported to Memorial Hospital for a doctor’s evaluation following the accident.
Mr. Flores-Ulloa was transported to Chatham County Detention Center and received a Pooler Municipal Court date of May 7, 2026, at 10AM. †Mr. Flores-Ulloa received a $ 2,500.00 bond.
Mr. Flores-Ulloa gave the names of the two other passengers as [WITNESS #1] and [REDACTED]. Searches for the other men had begun, but they had not been found. Mr. Flores-Ulloa stated [WITNESS #1] was the driver, [REDACTED] was in the passenger seat, and he was in the rear passenger seat.
The vehicle Mr. Flores-Ulloa had been in had a Massachusetts tag, [REDACTED], which did not come back to the Toyota. The VIN for the Toyota came back as showing the vehicle had no insurance and was not registered. An open Modela can was found in the vehicle. Mr. Flores-Ulloa had his non-stamped passport but did not have a driver’s license. Mr. Flores-Ulloa told the nurse that he had been drinking alcohol and had used cocaine today.
Mr. [WITNESS #1] was found a few hours after the accident with an injured leg that he stated did not happen in the accident (see accident crn 260217012). APO Samaniego investigated that accident.
Mr. [WITNESS #1] was brought to Memorial Hospital by EMS. His date of birth is [REDACTED]. Mr. [REDACTED] was not found.
The Massachusetts tag that did not belong to the vehicle, the driver, nor any of the passengers was removed from the vehicle and placed in locker number 004.

[NARRATIVE #2 on February 17, 2026]
On 02/17/2026 at approximately 3:30 a.m., I responded to Memorial Hospital to follow up with Mr. [WITNESS #1] (listed witness/vehicle occupant). Upon arrival, I made contact with Mr. [WITNESS #1] in Emergency Room Alpha 8. An ER nurse advised that Mr. [WITNESS #1] sustained three fractures to his right leg and was scheduled to undergo surgery at approximately 11:00 a.m.
Mr. [WITNESS #1] was transported to the hospital via EMS after an unknown individual called 911 in reference to a suspicious person observed on the I-95 Exit 104 bridge. The caller advised that the individual appeared to be attempting suicide by leaning against the bridge wall. It appeared that Mr. [WITNESS #1] had crawled from the original crash scene to Exit 104 due to his broken leg.
I interviewed Mr. [WITNESS #1] in reference to the collision. He appeared to be intoxicated, as there was a strong odor of an alcoholic beverage emitting from his person. Mr. [WITNESS #1] stated that he was the driver of the vehicle at the time of the crash. He advised that after the collision, he exited the vehicle and was subsequently struck by an unknown vehicle, which caused the fractures to his right leg.
Mr. [WITNESS #1] further stated that after being struck, he hopped over the guardrail and walked across the southbound lanes to the shoulder.
Mr. [WITNESS #1] was unable to provide his residential address in Orlando, Florida. He also stated that he did not have a phone and was unable to provide his contact number or any family members’ contact numbers. Mr. [WITNESS #1] advised that he was traveling with Mr. Junior to North Carolina at the time of the collision and confirmed that there were only two occupants in the vehicle at the time of the crash.
Mr. [WITNESS #1] was provided with the case report number.
