On date and time listed above, I Deputy J. Claxton was dispatched to the Pentecostal Worship Center (116 Lake Wanda Reita Road Summerville, Georgia 30747) in reference to a report of child abuse. When Deputy Reynold’s and I arrived on scene we met with the complainant and the mother of the juvenile victim (later identifed as [WITNESS #1]) she stated that she let her Juvenile stay at her sister ([WITNESS #2]) house because she had to work. She further stated that the Juveniles father (Matthew McGuire) went to the residence to visit with the Juvenile.

While he was there Mrs. [WITNESS #2] started “Blowing her phone up” stating that Mr. McGuire smacked the Juvenile in the face and grabbed him by the upper torso and pushed him against the brick wall outside the apartment complex, she also told her that Mr. McGuire was heavily intoxicated.

We then went to Lake Wanda Reita apartments where we met with the Juvenile and Mrs. [WITNESS #2]. I started speaking Mrs. [WITNESS #2] and she stated that she witness Mr. McGuire and the Juvenile was playing and acting like they was boxing. When the Juvenile hit Mr. McGuire he got visibly upset and hit him back harder. The Juvenile then hit him again in a playing manner thats when Mr. McGuire smacked the Juvenile in the face with enough force to leave a hand print and picked him up and pushed him against the wall.

I asked if I could see the Juvenile. The Juvenile walked outside and was shy and skiddish at first. I then asked if I could see his arm and face area. The Juvenile had a red mark on his right side of the face and a red mark on his left arm.

At this time, Deputy Reynold’s advised that he would be notifying DFCS. There was a crowd of people in front of the apartment and I asked the crowd if they saw this incident occurred and all persons present stated they did and they would be willing to fill out a voluntary witness statements of the event. They all agreed to do so and they all collaborated with the main witness [WITNESS #2].

At this time EMS personal arrived on scene and started checking on the Juvenile. I asked Mrs. [WITNESS #1] if I could take some pictures of the Juvenile marks. Mrs. [WITNESS #1] agreed and I began taking pictures of red mark on the Juveniles neck area. Mrs. [WITNESS #1] asked the Juvenile what happen and the Juvenile stated that “Daddy picked me up right there” and placed his hand on his chest and back area.

At this time, I started getting all of the witness statement forms back from the eye witnesses.

At this time, I contacted the on call Investigator D. Sanford and spoke to him about the investigation. Investigator Sanford stated that he would reach out to Mrs. [WITNESS #1] to set up further investigation.
