Additional remarks:
Due to the noted violations and the grade received, the facility shall receive follow-up(s) within the required time frames. Due to the required score, the department requests that a risk control plan be conducted to discuss long term compliance. Contact the inspector to schedule this meeting.
Due to the numerous violations and imminent health hazard (pest infestation) the facility opted to voluntarily close. A walk-through will be conducted on Monday, 6/8/2026, to ensure facility has been thoroughly cleaned and pest control application. prior to reopening.
