[NARRATIVE #1 on June 01, 2025]

On 06/01/25 at 17:04 hours, I OFC B. Smith responded to the area of SR 53 and Willow Rd in reference to a male who was jumping into traffic ahead of oncoming cars.

I arrived on scene and observed a black male (later identified as Michael Curtis) standing in the southbound lanes of SR 53. I got out and instructed the male to step to the shoulder of the road so we could talk. I advised other officers enroute to respond in Emergency Fashion due to the male being irrate and potentially becoming violent. I also believed him to be actively placing public safety at risk due to his behaviors and actions. He continuously tried to run back into the highway. I asked for EMS because he said his sister had poisoned him. He was being very belligerent and non compliant. The male kept trying to approach me and I kept giving verbal commands for him to step back. He complied to a certain extent but I had to repeatedly tell him to get back. I did observe a large bite mark on his sisters left side from where he bit her. PFC Evans, PFC Lanier and CPT Johnson arrived on scene and EMS arrived and we were able to get him to go to the hospital.

I began to talk to his family while CPT Johnson and PFC Lanier escorted him to the ambulance. They began to follow the ambulance to Floyd Hospital. I then heard CPT Johnson and PFC Lanier advise the male was trying to jump out of the ambulance. Myself and PFC Evans responded to SR 53 at Broadus Rd where they were located at. We arrived on scene and observed them both actively fighting the male who was physically resisting and being combative. He repeatedly attempted to pull away in an attempt to enter the roadway again. I grabbed his left arm and began to try and move his left arm behind his back so that he could be placed into handcuffs. I heard PFC Evans tell the male to quit resisting or he was going to be tased. He kept actively fighting us and was tased.

The male continued to fight and trying to harm other officers. I continued fighting with him in an attempt to place him into handcuffs to be restrained. He continued to offer physical violence to all officers on scene. He bit CPT Johnson on the hand causing severe injury to his hand. He was tased again by PFC Evans two additional times after still failing to comply to our lawful commands to stop resisting. He was still a risk to all officers and the general public because of his violence. He was finally able to be restrained into handcuffs behind the back. PFC Evans was able to effectively place leg restraints on the male as well but he still continued to try and resist all officers. I placed him into the backseat of patrol vehicle 40 where he was transported to Floyd Hospital and released into their custody. Floyd Hospital placed a 10-13 order on the male who was still physically resisting.

Warrants to be secured for the male for Felony Obstruction and Aggravated Battery against a Police Officer

[NARRATIVE #2 on June 01, 2025]

On the date and time listed above I Pfc Evans was dispatched to SR 53 to assist Officer Smith with a subject who was reportedly seen trying to jump in front of cars. Shortly after I arrived Mr. Curtis volunteered to be transported by EMS to Floyd Hospital because his sister poisoned him. He was profusely sweating and acting belligerent. I assumed since he wanted to go to the hospital he was going to calm down. Cpt. Johnson and Pfc Lanier began to follow EMS. His sister showed us a large bite mark on her left side where he bit her after becoming agitated.

I began to leave the scene when I heard Capt. Johnson say EMS was pulling over and he was trying to jump out. I drove emergency traffic to assist the officers. When I arrived I saw Capt. Johnson and Pfc Lanier trying to hold him on the ground. It looked like he was trying to fight and actively resist, also get up and run into traffic. To prevent him from endangering the public, the officers lives and his life I assisted them in preventing him from running into traffic. We were struggling to gain control of his arms which he was using to push him self up off of the ground. He was given the legal commands to place his hands behind his back which he refused. I drew my department issued taser and tased him in the right side of his back and drove the taser into his right calf to cause his involuntary muscle spasms and locked his muscles up. They were able to partially gain control of his hand.

After the first 5 seconds were over I waited to see if he would comply with the legal commands that he was given. He did not and he began to try to get back up again. I delivered another 5 seconds which allowed more control to be gained. After that 5 Seconds was up he again began to try to get up which continued to risk the safety of every one involved due to the high volume of traffic just feet away from us. I delivered another 5 Seconds from my taser which allowed officers to place both hands in handcuffs. I deployed my taser 3 times before he was safely detained. I then retrieved the leg restraints from my patrol car and placed him in them and double locked them for a safe and secure fit. He was escorted the front of patrol car 40 and I pulled the taser probes out of his back and placed them needle down in the cartridges all while he voluntary kicked my legs. He was placed in the back seat of patrol car 40 and transported to the Floyd Medical Center.

[NARRATIVE #3 on June 01, 2025]

On above date and time, I PFC Lanier and Capt. Johnson responded to Hwy 53 at Willow Rd to back up Officer Smith. Upon our arrival we observed Officer Smith speaking with Michael Curtis. He appeared to be under the influence of a possible illegal substance, he was sweating profusely and had pin point eyes. He was reportedly trying to jump in front of traffic placing the publics safety at risk as well as his own. During this time, Curtis advised he wanted to be transported with EMS to the hospital due to him stating he was poisoned by his sister. At this time, Capt. Johnson and myself advised E911 we would be following EMS to Floyd medical due to how Curtis was acting.

While traveling to Floyd ER we observed Floyd EMS pulling onto the shoulder of the roadway as if something was wrong. I then pulled behind them advising E911 of our location. I began to exit my patrol vehicle and observed Curtis jump toward Capt Johnson from the back of the Ambulance. I ran up to Capt. Johnson to assist in restraining Curtis. I grabbed Curtis while he began to actively resist Capt. Johnson. I began to try an hold the arm of Curtis feeling him tense up and pull away from me and attempting to run into traffic which placed the public, officers and Mr. Curtis’s safety at risk. I fell back into the Ambulance and made the decision to take Curtis to the ground by grabbing his waist and took him to the ground to keep him and us away from traffic.

Once Curtis was taken to the ground he began to fight myself and Capt. Johnson. Curtis began to bite Capt. Johnson and bite/kick us while he would not give us his arms to place him in hand cuffs. Curtis still actively biting Capt. Johnson, I pulled my Dept. Issued Taser and deployed a close contact deployment from my Taser to the back of Curtis. Observing this was still not working due to a bad connection, I connected my Taser to the back of his left leg and activated it once again. Curtis still actively fighting I activated the Taser to his leg one more time in attempt to restrain him and keep him from harming Capt. Johnson and myself, During this time, My Taser was knocked out of my hand and went into the grass area next to where we were fighting. During the altercation EMS Employee grabbed my hand cuffs from my back case and placed a single cuff to the left arm of Curtis. I began to manipulate the left arm of curtis while he still pulled away from me.

PFC. Evans and Officer Smith arrived to our location. Smith began to assist me with attempting to gain control of his left arm. PFC Evans deployed his Taser 3 times allowing me enough time, to safely place Curtis in hand cuffs and double locked them for safety.

Once Curtis was placed in hand cuffs he was placed in the back seat of Car 40 where he was transported to Floyd ER for treatment. While at Floyd ER Myself and Capt. Johnson was treated for the Injuries we received while fighting Curtis. I received Injuries to my right arm and left arm as well as a left shoulder complaint that will need a follow up. Both knees received injuries as well. Also during the altercation my Taser was damaged, Ray Ban sunglasses were broken, Dept Ball Cap Damaged, as well as my Apple Watch face was broken.

[NARRATIVE #4 on June 01, 2025]

On 06-01-2025 at 1704 hours officers responded to Hwy 53 near the intersection Willow road due to Male subject running out in traffic on Hwy 53. At one point the subject jumped on the hood of one persons car and then tried to jump onto another car before Officer Smith arrived. Upon arrival Smith was able to get Curtis to stand by his patrol vehicle until EMS arrived. When I arrived, Curtis, Was standing next to Officer Smith explaining to him that his sister had been trying to poison him. Curtis was sweating profusely and speaking rapidly stating the same things over and over and attempted to walk away to wards traffic at one point. Curtis also appeared to have been bleeding from his nose. Smith Advised that Curtis had been with his family who were attempting to get him some help when he jumped out of there vehicle and started jumping onto other vehicles. Smith Also advised that he had bit his sister leaving visible damage to her.

When EMS arrived Curtis agreed to go into their care so that he could get some help. Officers escorted him to the Ambulance where he was assisted by medics. EMS asked if officers could follow them to the hospital due to his erratic behavior. Officer Lanier and I followed EMS toward the hospital.

After about a mile we observed the back door of the ambulance shutter and the ambulance immediately pulled off to the right of the road. We pulled in behind them in order to assist as needed. As I approached the back door, it immediately popped open and Curtis jumped out toward me and started trying to run toward the Hwy. In order to prevent Curtis from running into traffic I attempted to grab him and push him in the direction of the grass, away from the road. Curtis pulled away continuing in the direction of the Hwy. I then attempted to wrap my arm around his head and guide him to the ground but he was still able to push us closer to the road. Officer Lanier was able to wrap around his lower body and pulled him the ground.

Curtis did not stop trying to get back up and pull away. We gave him numerous commands to stop. At this point he started to shove us off him and bit me numerous times on the left arm and right hand leaving visible damage to my arm and hand. At one point he bit deeply into my right middle finger and used his strength drive my right hand into the gravel while still attached to my finger. He also tried to bite Officer Lanier on several occasions. I struck him numerous times in the right side of the head near his right ear in attempt to get him to stop biting me and keep his mouth away from my arm. This was not effective he tried many more times to bite me and other officers. Officer Lanier deployed his taser but it was ineffective, Curtis continued to lift us off the ground. When officers Evans and Smith Arrived, Evans deployed his taser to the right side of Curtis’s back and after a few cycles we were finally able to get him to lie flat and pry his arms around to his read in order to apply cuffs. Even after being cuffed Curtis attempted to kick at us a few more times before finally complying with commands.

Curtis was handcuffed properly and double locked before being placed in the back seat of Smith’s Patrol car. EMS was asked to follow us to the hospital in case Curtis experienced any episodes of excited delirium. Once at the hospital security had to physical restrain him until medication to be given to him in order for him to calm down Floyd Medical placed a 10-13 hold on Curtis and Officers Smith will be taking out arrest warrants at a later time.

Officer Lanier and myself were checked by Floyd Medical for injuries received.
