On Thursday, October 16, 2025, Capt. Shepard recieved a message to contact [REDACTED] with the D.A.’s office at approximately 08:47. I contacted Assistant District Attorney [REDACTED] by phone at approximately 08:52 who stated he needed an escape warrant on Robert Howell Slade. Mr. Robert continued by saying Slade was suppose to turn himself in no later than Wednesday, October 15, 2025, according to his sentence.
I told Mr. [REDACTED] I had no problem taking the warrants but I would need the paperwork from his sentencing sent to me in order to take out the warrant. Mr. [REDACTED] stated he understood and was going into court and would have Investigator Vaughn send me the paperwork.
Investigator Vaughn called me at approximately 12:32 and told me he sent me his sentencing paperwork. I told Investigator Vaughn I would return to my office and complete the warrant. After arriving at my office, I completed the necessary paperwork for the arrest warrant on Slade for the offense of escape. I swore to the warrant at 13:07 and then notified Investigator Vaughn the warrant was signed.
I asked Detective Shelton to send the information on Slade out to his group. He stated he would. Investigator Vaughn stated he already filled out the paperwork for an offline search for Slade’s registered vehicles. I ensured dispatch entered the warrant into G.C.I.C. and sent a statewide A.M. message as a bolo for Slade.
After returning to the office, I ran Slade’s vehicle through FLOCK, his registered vehicle is a 2015 GMC Sierra, white in color, GA tag: [REDACTED]. The last flock hit was on his truck was in Gwinnett County at the intersection of Buford Dam Rd. and South Waterworks Rd. towards Shadburn Ferry.
At approximately 20:11, I received a text from Investigator Greene to Investigator Vaughn and myself letting us know Slade was in Northside Forsyth Hospital on a 10-13. I told him I did not know if he could be released to us with a 10-13 signed. Investigator Greene said he would find out more information. Investigator Greene forwarded us the e-mail from SSGT Moore with Hall County Sheriff’s Office who stated he was in Northside Forsyth.
On Friday, October 17, 2025 at approximately 07:39, I called Northside Hospital Forsyth County and they verified Slade was in the hospital in the Emergency Department, ED 31. They told me he has been there 16 hours, which means he arrived around 15:39 on October 16, 2025. They also told me he has been accepted to Highland Rivers Cedartown and they would need a court order for the release.
I called and texted to send updates to the Chief Deputy, Investigator Greene and District Attorney [REDACTED].
At approximately 07:59, I called Deputy Ryan Scott and asked him if he could get ready and come to the office.
After getting of the phone with Deputy Scott, I called the Forsyth County Dispatch non-emergency number at approximately 08:00. I informed them of the situation and the dispatcher stated she would have the Cumming P.D. supervisor call me and she placed me on hold.
At approximately 08:02, I was connected with Sgt. Burns with Cumming P.D. Sgt. Burns said they would go to the hospital and try to take Slade into custody but did not know how the hospital would react.
At approximately 08:25, I spoke with Sgt. Burns again and he said the hospital would cooperate if I would send the court orders and he texted me his e-mail address. I sent him the court order for detention and the escape warrant.
I called the Chief Deputy, Investigator Greene and Capt. Williams and advised them of the update. I advised Capt. Williams we would need to keep Slade on suicide watch and possibly get him a mental health evaluation. She stated she would call the state and try and expidite the process for him to get to GDCP.
At approximately 09:00 Sgt. Burns called and said his guys would be taking Slade into custody and asked if we could meet them. I told him we would meet them. He stated he would call back when they were en-route with Slade.
He called back a few minutes later saying his officer was on the way to Dahlonega, GA to meet. I then sent Deputy Scott and Sgt. Farnham to meet his officer and take custody of Slade.
I then called the magistrate office to try and arrange first appearance and was told a judge would call me back because they were in court at the moment.
Sgt. Farnham and Deputy Scott arrived in Dahlonega at approximately 10:34 and took custody of Slade. Slade was searched, handcuffed and double locked and the transport vehicle was searched prior to their departure.
Deputy Scott and Sgt. Farnham arrived at the detention center at approximately 11:53 and turned Slade over to detention personnel after reminding them of the suicide watch. The transport vehicle was searched after transport.
