The violations noted by the inspector were:

Violation #1:
1-2A: .03(2)(a)-(n),(p)Several risk factor violations occurred today during inspection due to lack of active managerial control. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2:
4-1A: .04(4)(n)1Observed employee touched raw hamburger meat and proceeded to touch fryer, hamburger bun, and to-go container without properly disposal of gloves and washing hands. If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. (P)
Corrective Actions: Buns and to-go container discarded. Handles sanitized and employee removed gloves and washed hands.

Violation #3:
4-2B: .05(7)(a)1 Observed several knives, whisks, and cutting appliances with food debris. Ice machine observed with build up inside bin. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

Violation #4:
6-1C: .04(6)(d)Observed multiple rice containers not cooling at fast enough rate; see temp log. TCS foods being cooled from 135°F to 70°F in 2 hours MUST cool 0.54°F per minute (roughly 32.5°F per hour). TCS foods being cooled from 70°F to 41°F MUST cool 7.25°F per hour (0.121°F per min or 29°F in 4 hours).
Corrective Actions: Cooling methods adjusted to cool down faster.

Violation #5:
8-2B: .07(6)(g)Observed chlorine sanitizer in sink too high; above 100 ppm. Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials used on food contact surfaces shall not exceed stated concentrations as listed on the EPA-registered label, in 40 CFR 180.940 and 40 CFR 180.2020, (100 ppm for chlorine and 400 ppm for quaternary ammonia). (P)
Corrective Actions: PIC diluted sanitizer in sink to safe concentration.

Violation #6:
11A: .04(6)(e)Multiple rice containers cooling down with lids and stacked on top of each other. Foods shall be cooled using one or more methods approved by the health authority (e.g., smaller batches, thinner portions, using rapid cooling equipment, stirring food in containers placed in an ice water bath, using containers that facilitate heat transfer and ice as an ingredient; placing items in the walk-in cooler or walk-in freezer partially or completely uncovered as long as the food is protected from overhead contamination). (Pf, C)
Corrective Actions: Rice uncovered and placed in walk in freezer to cool down faster.

Violation #7:
12C: .04(4)(m)Observed wiping cloths in 2 chlorine sanitizer buckets below 50 ppm. Cloths in-use for wiping counters and other equipment surfaces shall be stored in sanitizer of the appropriate concentration in-between uses and laundered daily. (C)
Corrective Actions: PIC remade 2 chlorine sanitizer buckets to safe concentration levels

Violation #8:
14C: .05(6)(r)Observed garlic butter wing sauce stored in a single use container and not stored in its original container; observed multiple single use containers reused to store honey in walk-in cooler. Single-service and single-use articles may not be reused. (C)

Violation #9:
15A: .05(6)(a)Observed prep table in disrepair. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)

Additional remarks:
All cold holding are in compliance unless noted otherwise.

A follow inspection will occur within 10 days. Correct any violations or permit will result in suspension.

A required additional routine inspection will occur within 12 months.

2 Consecutive Unsatisfactory Scores will result in permit suspension.
