The violations noted by the inspector were:
Violation #1:
1-2A: .03(2)(a)-(n),(p)Observed multiple risk factor violations during today’s inspection resulting in an unsatisfactory score. The person-in-charge is not practicing active managerial control of the facility. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)
Violation #2:
1-2B: .03(3)(c)Facility’s Certified Food Safety Manager (CFSM) certificate is not posted. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. (Pf)

Violation #3:
2-1B: .03(5)(c)Observed employee enter kitchen, put gloves on, and start to make an order without washing hands. Employees must wash hands and exposed portions of arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service articles, and: 1) after touching exposed body parts; 2) after using the toilet room; 3) After caring for or handling service animals or aquatic animals; 4)after coughing, sneezing, using tobacco, eating, or drinking; 5) after handling soiled equipment or utensils; 6) during food preparation, as often as needed to remove soil and contamination and to prevent cross contamination when changing tasks; 7) when switching between working with raw food and working with ready-to-eat food; 8) before putting on gloves; 9) after engaging in other activities that contaminate hands; 10) a second time when entering the kitchen from using the restroom. Corrective Actions: Employee removed gloves, washed hands, and put on new gloves.
Violation #4:
2-2A: .03(2)(o)Person-in-charge (PIC) could not locate signed employee health agreement for only employee onsite. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf)
Corrective Actions: Employee signed employee health agreement.
Violation #5:
4-1A: .04(4)(c)1(i)(ii)(iii)(v) (vi)(vii)(viii)Observed raw ground beef stored above ready to eat bread and raw chicken stored above ready-to-eat chicken in reach-in cooler. Observed raw fish stored above ready-to-eat fries and raw cheesesteak meat stored above ready-to-eat mozzarella sticks in reach-in freezer. Food shall be protected from cross contamination during display, storage, and preparation. (P)
Corrective Actions: Items were placed in proper storage areas.
Violation #6:
4-2B: .05(7)(a)1Observed microwave with significant food debris inside. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)
Corrective Actions: Microwave was cleaned.
Violation #7:
6-1A: .04(6)(f)Observed multiple time/temperature control for safety foods holding at temperatures above 41F (see temperature log). Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: Items were discarded.
Violation #8:
6-1C: .04(6)(d)Observed multiple mango lassi cups that were not able to cool within 4 hours. TCS foods being cooled from 135°F to 70°F in 2 hours MUST cool 0.54°F per minute (roughly 32.5°F per hour). TCS foods being cooled from 70°F to 41°F MUST cool 7.25°F per hour (0.121°F per min or 29°F in 4 hours).
Corrective Actions: Mango lassis were discarded.
Violation #9:
6-2: .04(6)(g)Observed multiple containers of milk, containers of precooked chicken, and container of nacho cheese opened more than 24 hours ago without date marks. Refrigerated, ready-to-eat, time/temperature control for safety food prepared and packaged by a food processing plant shall be clearly marked, at the time the original container is opened in a food service establishment and if the food is held for more than 24 hours, to indicate the date by which the food shall be consumed, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days after the original container is opened. The day the original container is opened shall be counted as Day 1. Date marking by the food service establishment may not exceed a manufacturer’s use by date if the manufacturer determined the use-by date based on food safety. (Pf)

Violation #10:
10D: .04(7)(d)Facility does not have allergen disclosure either in customer view or on menu. 1. If required by law, consumer warnings shall be provided. (C) 2. Food service establishment or manufacturers’ dating information on foods may not be concealed or altered. (C) 3. The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. (C)
Violation #11:
11C: .04(6)(c)Observed containers of chicken thawing at room temperature in kitchen. Observed container of fish thawing while still in vacuum packaging. Time/temperature control for safety food shall be thawed: 1) under refrigeration that keeps the food at 41°F or less; 2) completely submerged in running water with the temperature of the water at 70°F or less, has sufficient flow to agitate and float off loose particles in an overflow, and that no part of the food exceeds 41°F; 3) as a part of the cooking process. (C) Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment: (i) Prior to its thawing under refrigeration that maintains the food temperature at 41°F (5°C) or less; (C) or (ii) Prior to, or immediately upon completion of its thawing, using procedures to completely submerge in running water. (C)
Corrective Actions: Chicken was placed into cooler to thaw. Fish was discarded.
Violation #12:
12B: .03(5)(j)Observed PIC handle food without hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2″). (C)
Corrective Actions: PIC put on hairnet.
Violation #13:
15A: .05(6)(a)Reach-in cooler in back storage area is not able to hold foods at 41F or below. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)
Violation #14:
15B: .05(3)(h),(i)Facility has quat sanitizer at sink and Purell sanitizer for tables, but does not have test strips for them. Facility has chlorine test strips but no chlorine sanitizer. A test kit or other device that accurately measures the concentration in mg/L (or ppm) of sanitizing solutions shall be provided. (Pf)
Violation #15:
17D: .07(3)(f)No light in dry storage area. The light intensity shall be at least 10 foot candles in walk-in refrigeration units and dry storage areas and other rooms during periods of cleaning. (C)
