Gangnam Style Karaoke, located at 2645 N Berkeley Lake Rd NW, Suite D 228 in Duluth, received a score of 58 points out of a possible 100 points during their 04/27/2026 inspection by the Georgia Department of Public Health.

The violations noted by the inspector were:

Violation #1:
1-2A: .03(2)(a)-(n),(p)Several risk factor violations were observed during today’s inspection resulting in an unsatisfactory score as a result of an overall lack of active managerial control.// It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2:
2-2A: .03(1)(c)2,3,17Person in charge was not aware of all reportable symptoms and illnesses.// Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissible through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. (Pf)
Corrective Actions: Person in charge was educated on all reportable symptoms and illnesses.//

Violation #3:
2-2D: .07(3)(b)Observed handwashing sinks in men’s restroom with no paper towels. No hot air dryer is present. ***SECOND CONSECUTIVE VIOLATION***// Each handwashing sink shall be provided with individual, disposable towels,; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that delivers high velocity, pressurized air at ambient temperatures. (Pf)
Corrective Actions: Paper towels were placed in restroom.//

Violation #4:
2-2D: .07(3)(a)Observed handwashing sinks in men’s restroom with no soap available.// Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. (Pf)
Corrective Actions: Soap was placed in restroom during inspection.//

Violation #5:
3-1C: .04(1)Observed the following foods in walk-in cooler with mold growth: half and half, tomato sauce, cucumber, salted shrimp, cream cheese, canned peaches, rice, rice cakes, and two unidentifiable foods.// Food shall be safe, unadulterated, and honestly presented. (P)
Corrective Actions: Food was discarded.//

Violation #6:
4-2B: .05(7)(a)1Observed black mold-like accumulation on interior food-contact surfaces of ice machine.// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

Violation #7:
6-1A: .04(6)(f)Observed all time/temperature control for safety foods in walk-in cooler holding above 41F (see temperature log).// Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: Food was discarded.//

Violation #8:
10D: .04(8)(b)Observed four packages of tofu with printed expiration dates of 4/6/2026. Observed salted shrimp with a printed expiration date of 4/15/2026. Obserced sliced rice cakes with a printed expiration date of 2/27/2026. Observed two packages of sliced cheese with printed expiration dates of 1/28/2025.// Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. (P)
Corrective Actions: Foods were discarded.//

Violation #9:
15A: .05(6)(a)Observed walk-in cooler unable to maintain proper cold holding temperatures. Minimum ambient temperature was 65F.// Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)

Violation #10:
15B: .05(6)(d),(e)Observed warewashing machine dispensing less than 50ppm chlorine.// A warewashing machine and its auxiliary components shall be operated in accordance with the machine’s data plate and other manufacturer’s instructions; and its conveyor speed or automatic cycle times shall be maintained accurately and timed in accordance with the manufacturer’s specifications. (C)

Violation #11:
17A: .07(2)(l)Observed men’s restroom door is not self-closing. Self-closing mechanism is broken.// Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. (C)

Violation #12:
17A: .07(5)(i)Observed men’s restroom door open. No cleaning or maintenance was occurring. ***SECOND CONSECUTIVE VIOLATION***// Except during cleaning and maintenance operations, toilet room doors shall be kept closed. (C)

Violation #13:
18: .07(2)(m)Observed front door and back door propped open at beginning of inspection while not actively being used. ***SECOND CONSECUTIVE VIOLATION***// Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; (C) 2) closed, tight-fitting windows; (C) and 3) solid self-closing, tight-fitting doors. (C)
Corrective Actions: Doors were closed.//

Additional remarks:
Due to today’s score, a follow-up inspection will occur within 10 days. A required additional routine inspection will occur within 12 months.

Repeat violations observed today. Three consecutive violations of the same code provision on routine inspections will result in a permit suspension.

All cold holding temperatures in compliance unless otherwise stated.
