Hibachi Grill And Supreme Buffet, located at 419 Pike Blvd, Lawrenceville, GA 30046, received a score of 59 points out of a possible 100 points during their 08/19/2026 inspection by the Georgia Department of Public Health.

The violations noted by the inspector were:

Violation #1:
1-2A: .03(2)(a)-(n),(p)Multiple risk factor violations were observed during today’s inspection that were not in compliance with the Food Code. As a result, the facility received an unsatisfactory score, indicating that the Person in Charge (PIC) is not maintaining adequate active managerial control of the facility.// It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2:
2-2A: .03(2)(o)Observed that the person in charge could not demonstrate in a verifiable manner that the food employees were informed of their responsibility to report symptoms and/or illnesses.// Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf)

Violation #3:
2-2B: .03(5)(k)1&2Observed employee drinks in unapproved containers (soda cans, disposable cup with lid but no straw) stored on shelves above the ware washing area and the 3-compartment sink.// Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). (C)
Corrective Actions: Employee drinks were discarded.

Violation #4:
2-2D: .06(2)(o)Observed an employee filling up a sanitizer bucket with water from the hand sink located at the front hibachi station.// A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer’s instructions. (Pf)
Corrective Actions: Employee was instructed to fill up the sanitizer bucket in a sink from a service sink.//

Violation #5:
3-1A: .04(2)(a)Observed that the facility is purchasing the molluscan shellfish from a source that is not on the ICSSL approved list for shellfish reshippers.// Food shall be obtained from sources that comply with law. (P)
Corrective Actions: The five boxes of New Zealand Mussels were discarded.//

Violation #6:
4-2B: .05(7)(a)1Observed the ice machine to have a black mold like substance in the lower chamber. ***THIRD CONSECUTIVE VIOLATION OF THE CODE PROVISION***// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

Violation #7:
6-1A: .04(6)(f)Observed multiple time-temperature control for safety (TCS) foods holding temperature above 41F (see temperature log).// Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: Foods were discarded.//

Violation #8:
8-2B: .07(6)(c)Observed butane lighters stored above the open top prep table in the kitchen.// Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by: 1. Separating the poisonous or toxic materials by spacing or partitioning; (P) and 2. Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles. (P)
Corrective Actions: The butane lighters were removed and stored below any food or single use items.//

Violation #9:
8-2B: .07(6)(b)Observed three containers of sanitizer that were not labeled with the common chemical name.// Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. (Pf)
Corrective Actions: Person in charge labeled the sanitizer buckets with the common name.//

Violation #10:
15A: .05(6)(a)Observed an upright cooler in the hibachi station and a walk-in cooler holding temperatures above 41F.// Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)

Violation #11:
17D: .07(4)(b)Observed container of personal items ( lipstick, vaseline, and car keys) stored on top of a food prep table. Also observed a purse, keys and phone stored above the open top prep cooler in the kitchen.// Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. (C)
Corrective Actions: The person in charge moved all personal items so that they are beneath food or single use items.//

Violation #12:
18: .07(2)(m)Observed that the screen door that is currently being used on the back door is not sealed at the bottom threshold.// Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; (C) 2) closed, tight-fitting windows; (C) and 3) solid self-closing, tight-fitting doors. (C)

Additional remarks:
Note: Today’s inspection is a required additional routine inspection (RAR). Another routine inspection will occur on or before December 31, 2026.

Note: All cold-holding and hot-holding items were in compliance unless otherwise noted.

Additives: red tomato and egg yellow shade.

Note: Grout is degraded on tiles located at the server station. Ensure grout is replaced in the next 6 months.

Note: Ensure that all walls, floors, and ceiling are cleaned routinely.

Note: Ensure that any ceiling tiles moved out of place for repair in the kitchen are put back in place after repair is complete.

**NOTE: Permit suspended due to Third consecutive violation of item 4-2B: [.05(7)(a)1] – Equipment, Food-Contact Surfaces, and Utensils. Permit was reinstated after on-site training was provided.

**Note: A follow-up inspection will occur by 8/28/2026. Ensure all violations are corrected, or the food service permit may be suspended.

Note: A required additional inspection will occur within the next 12 months.
