The violations noted by the inspector were:
Violation #1:
1-2A: .03(2)(a)-(n),(p)Several risk factor violations were observed during today’s inspection resulting in an unsatisfactory score as a result of an overall lack of active managerial control. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2:
4-1A: .04(4)(c)1(i)(ii)(iii)(v) (vi)(vii)(viii)Raw chicken stored over raw pork in upright freezer and in another upright freezer, raw ground beef stored over cooked mushrooms. Food shall be protected from cross contamination during display, storage, and preparation. (P)
Corrective Actions: PIC (person in charge) had items rearranged to store appropriately.
Violation #3:
6-1A: .04(6)(f)Observed multiple TCS (time/temperature control for safety) items holding above 41F. See temperature log. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: PIC discarded food. ***THIRD CONSECUTIVE VIOLATION OF SAME CODE PROVISION***
Violation #4:
6-2: .04(6)(g)Observed nine pans of cooked pork dumplings in the upright freezer (cooked 2 days ago) that did not have the date prepared on them. Squid salad in front upright cooler that was made 3 days ago without a date mark. Refrigerated, ready-to-eat, time/temperature control for safety foods prepared and held in a food establishment for more than 24-hours shall be clearly marked to indicate the date of prep or the date by with the food shall be consumed, sold, or discarded when held at 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. (Pf)
Corrective Actions: PIC labeled items with datemark and discarded the squid salad. ***THIRD CONSECUTIVE REPEAT OF SAME CODE PROVISION***
Violation #5:
10D: .04(7)(d)Did not have an allergen disclosure statement indicating all allergens on site. 1. If required by law, consumer warnings shall be provided. (C) 2. Food service establishment or manufacturers’ dating information on foods may not be concealed or altered. (C) 3. The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. (C)

Violation #6:
15A: .05(6)(a)Observed prep cooler not holding below 41F. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)
Corrective Actions: TCS food was removed (and discarded) but could not correct on-site. Please correct by 8/28/2026.
Violation #7:
15C: .05(7)(a)2,3Observed build up under hood, on shelving of prep counters and in and on upright coolers. There was also build up on the back side on metal pans and on containers holding dry goods. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. (C)

Violation #8:
17C: .07(5)(a),(b)1,2,3Observed build up on floors and walls of whole facility. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)

Violation #9:
17D: .07(4)(b)Observed cell phone on prep counter. Employee food stored over customer food on shelving and in upright coolers and freezers. The employee food found over customer food was watermelon, popsicles and caviar. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C)
Corrective Actions: PIC moved items to store appropriately.

NOTE: All cold and hot held temperatures were in compliance unless otherwise noted.
NOTE: THIRD REPEAT VIOLATIONS RESULTED IN PERMIT SUSPENSION FOR 6-1A: COLD HOLDING AND 6-2: DATE MARKING. On-site training provided and permit was reinstated.
NOTE: Today’s inspection is an additional routine from the score below 80 from 8/20/2025. A regular routine will still be performed by December 2026.
NOTE: A follow up will be conducted within 10 days. Correct any uncorrected violations or permit may be suspended. Two consecutive unsatisfactory scores will result in permit suspension.
NOTE: A required additional routine will be conducted within 12 months.
