Oriental Garden, located at 4132 Athens Hwy, Loganville, GA 30052, received a score of 57 points out of a possible 100 points during their 09/30/2026 inspection by the Georgia Department of Public Health.
The violations noted by the inspector were:
Violation #1:
1-2A: .03(2)(a)-(n),(p)Several risk factor violations were observed during today’s inspection resulting in an unsatisfactory score as a result of an overall lack of active managerial control. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)
Violation #2:
4-1A: .04(4)(c)1(i)(ii)(iii)(v) (vi)(vii)(viii)Observed raw chicken being stored above raw beef in the walk in cooler, and raw shelled eggs being stored above green onions. In the walk in freezer raw pork was being stored above crab rangoons. Observed raw eggs being stored above ready to eat cabbage in prep top cooler. Observed lighter being stored above read to eat eggrolls. Food shall be protected from cross contamination by: (i) Except as specified in paragraph (i)(IV) of below, separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, (P) and (II) Cooked ready-to-eat food; and (P) (III) Fruits and vegetables before they are washed; (P) (IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food. (ii) Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by: (I) Using separate equipment for each type, (P) or (II) Arranging each type of food in equipment so that cross contamination of one type with another is prevented, (P) and (III) Preparing each type of food at different times or in separate areas; (P) (iii) Cleaning and sanitizing equipment and utensils; (v) Cleaning hermetically sealed containers of food of visible soil before opening; (vi) Protecting food containers that are received packaged together in a case or overwrap from cuts when the case or overwrap is opened; (vii) Storing damaged, spoiled, or recalled food being held in the food service establishment separate from food, equipment, utensils, linens and single-service and single-use articles; or (viii) Separating fruits and vegetables, before they are washed from ready-to-eat food.
Corrective Actions: Person in Charge (PIC) rearranged items in the cooler and freezer so that they were stored by cook temp.
Violation #3:
4-2B: .05(7)(a)1Observed ice machine with a build up of debris. Observed microwave with a build up of debris. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)
Corrective Actions: Could not correct on site. Correct by 10/09/2026.
Violation #4:
6-1A: .04(6)(f)Observed cut cabbage, bean sprouts, fried chicken, and pooled raw eggs temping above 41F. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: PIC moved cabbage, bean sprouts, and fried chicken to walk in cooler. Pooled raw eggs were discarded.
Violation #5:
6-1D: .04(6)(i)Observed container of beef, container of chicken, and noodles on time control without a start and end time label. When using time without temperature control (Time Control, TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below when removed from temperature control; (P) 2) the food items shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time the food was removed from temperature control; (Pf) 3) foods not cooked and served or consumed within 4 hours of being removed from time control shall be discarded; (P) 4) food in unmarked containers or packages, or marked to exceed a 4-hour limit shall be discarded; (P) 5) written procedures for TPHC (including methods of compliance with cooling foods is prepared, cooked, and refrigerated) shall be created in advance, kept in the food service establishment, and made available upon the Health Authorities request. (Pf)
Corrective Actions: PIC discarded items.
Violation #6:
6-2: .04(6)(g)Observed fried chicken wings made two days ago and cooked pork that was previously frozen but made a week ago in the prep cooler with no date marking. Observed 4 trays of crab rangoons in the walk in freezer that were made two days ago with no date marking. Observed 3 containers of cooked lo mein noodles in the walk-in cooler that were made yesterday with no date-marking. Refrigerated, ready-to-eat, time/temperature control for safety foods prepared and held in a food establishment for more than 24-hours shall be clearly marked to indicate the date of prep or the date by with the food shall be consumed, sold, or discarded when held at 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. (Pf)
Corrective Actions: PIC put date mark on all items.
Violation #7:
8-2B: .07(6)(g)Observed the only two buckets of bleach sanitizer solution (with cloths stored in them) at too high of a concentration. Chemical sanitizers, including chemical sanitizing solutions generated on-site, and other chemical antimicrobials used on food contact surfaces shall not exceed stated concentrations as listed on the EPA-registered label, in 40 CFR 180.940 and 40 CFR 180.2020, (100 ppm for chlorine and 400 ppm for quaternary ammonia). (P)
Corrective Actions: PIC added water to bleach sanitizer solution.
Violation #8:
14A: .04(4)(k)Observed scoops with no handle in direct contact with food. Observed 3 utensils being stored in water not 135F or above (see temp log). During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1. in the food with their handles above the top of the food and the container; 2. In food that is not time / temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed; 3. On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency; 4. in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 5. in a clean, dry container; or 6. in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at the required frequency. (C)
Corrective Actions: PIC removed scoops from food items. PIC removed utensils form water and cleaned them again. ***THIS IS THE SECOND CONSECUTIVE VIOLATION OF THE SAME CODE PROVISION****
Violation #9:
14C: .05(6)(r)Observed two single-use containers of soy sauce being reused, one to store rice and the other to store salt. Single-service and single-use articles may not be reused. (C)
Corrective Actions: PIC moved rice to appropriate container. Salt could not be moved to another container. Correct by 10/09/2026.
Violation #10:
15A: .05(1)(a)Observed cardboard being used as lining for shelves. Observed aluminum foil being used to line shelf. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions, and shall be: (P) safe (P), durable, corrosion-resistant, and nonabsorbent. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. (C)
Corrective Actions: PIC removed cardboard and aluminum foil.
Violation #11:
15C: .05(7)(a)2,3Observed shelved in walk in cooler with accumulation of debris. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (C)
Corrective Actions: Could not correct on site. Correct by 10/09/2026.
Violation #12:
16B: .06(2)(r)Observed handwashing sink with water turned off because of concern for leaks. A plumbing system shall be repaired according to law; (P) and maintained in good repair. (C)
Corrective Actions: PIC turned water back on. Will return on or by 10/09/2026 to ensure leak has been repaired.
Violation #13:
17C: .07(5)(a),(b)1,2,3Observed foil on wall to cover a pass through window. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)
Corrective Actions: Could not correct on site. Window must either be kept or remodeled with appropriate finishes. Correct by 10/09/2026.
Violation #14:
17D: .07(4)(b)Observed employee food and eyeglasses stored in food prep area. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C)
Corrective Actions: PIC moved items to designated employee area.
Violation #15:
18: .07(2)(m)Observed gaps in the bottom and top of screen door being used since back door was open. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; (C) 2) closed, tight-fitting windows; (C) and 3) solid self-closing, tight-fitting doors. (C)
Corrective Actions: Could not correct on site. Correct by 10/09/2026.
