Smoothie King, located at 3375 Sugarloaf Pkwy, Ste. 106, Lawrenceville, GA 30044, received a score of 61 points out of a possible 100 points during their 09/29/2026 inspection by the Georgia Department of Public Health.

The violations noted by the inspector were:

Violation #1:
1-2A: .03(2)(a)-(n),(p)Observed several risk factor violations today as a result of an overall lack of active managerial control.// It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2:
2-2A: .03(1)(c)2,3,17Person in charge was not able to demonstrate knowledge of all reportable symptoms and illnesses.// Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissible through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. (Pf)
Corrective Actions: Person in charge was educated on reportable symptoms and illnesses and provided with an employee health reference poster.//

Violation #3:
3-1C: .04(1)Observed mold-like growth on commercially frozen thawed pineapple and pumpkin puree. Observed unidentifiable black substance on several pieces of ice in ice machine.// Food shall be safe, unadulterated, and honestly presented. (P)
Corrective Actions: Food and ice was discarded.//

Violation #4:
4-2B: .05(7)(a)1Observed accumulation of black mold-like substance on interior surfaces of ice machine.// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

Violation #5:
6-1A: .04(6)(f)Observed several time/temperature control for safety foods cold holding above 41F in prep cooler (see temperature log).// Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: Food was discarded.//

Violation #6:
10D: .04(8)(b)Observed individually packaged cheeses with printed manufacturer’s expiration dates of 6/17/2026, 8/30/2026, and 9/14/2026.// Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. (P)
Corrective Actions: Cheese was discarded.//

Violation #7:
12D: .04(4)(g)Observed sticker on banana was not removed prior to being washed.// Raw fruits and vegetables shall be thoroughly washed in water, in a sink designated for that purpose only, to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in ready-to-eat form. (Pf)
Corrective Actions: Sticker was removed and banana was washed again.//

Violation #8:
13A: .02(1)(d)Observed the most recent inspection report is not posted. An old inspection report from 11/5/2025 is posted.// The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. (C)
Corrective Actions: Post today’s inspection report in public view.//

Violation #9:
15A: .05(6)(a)Observed prep cooler unable to maintain proper cold holding temperatures. Minimum ambient temperature was 74F. Observed ice accumulation around the gasket of the right side door of the standing freezer in the back of the kitchen.// Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)

Additional remarks:
Due to today’s score, a follow-up inspection will occur within 10 days. A required additional routine inspection will occur within 12 months.

Failure to correct violations within 10 days may result in permit suspension.

Note: Ensure disinfectant for vomit/diarrhea kit is approved. Disinfectant must be listed on EPA List G indicating it is effective against norovirus.

Note: Do not use prep cooler for cold holding foods until repaired. Discussed using time control for foods in prep cooler until repaired. Provided time control procedures to person in charge and discussed requirements.

All cold holding temperatures in compliance unless otherwise stated.
