Atlanta’s Smoothie King Receives 55% on Public Health Inspection

Smoothie King, located at 2068 HENDERSON MILL RD, received a score of 55 points out of a possible 100 points during their 09/17/2021 inspection by the Georgia Department of Public Health.

The violations noted by inspector Evangeline Reaves were:

Violation of Code: [511-6-1.03(5)(c)]
Observed employee prepping food and containers after exchange of money with customer in drive-thru. Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils. New Violation.

Violation of Code: [511-6-1.03(5)(j)1&2]
Employees shall consume food only in approved designated areas separate from food preparation and serving areas. Observed location of designated employee eating, drinking, or smoking area causing possible cross-contamination to paper towels. Informed employee that all drinking containers should be in designated employee area. New Violation.

Violation of Code: [511-6-1.04(4)(c)1(iv)]
Observed bananas , mango, strawberries and pineapple stored on top of counter uncovered and exposed to cross contamination of flies and fruit flies. Storing the food in packages, covered containers not observed at time of inspection. Informed PIC to cover all food item to prevent cross contamination from splashing water and flies. New Violation.

Violation of Code: [511-6-1.05(7)(b)]
Observed blender with heavy residue and dead bee on side of equipment. Blender not properly cleaned. Informed PIC to properly clean and sanitize all blenders and tops to blender after use. New Violation.

Violation of Code: [511-6-1.04(6)(f)]
Observed RIC not holding at 41 degrees required temperature. Informed PIC to lower temperature to maintain properly cold holding. At the time of inspection the temperature was 47 degrees. Advised to remove some of the items to RIC cooler. New Violation.

Violation of Code: [511-6-1.04(6)(g)]
Observed ready to eat food not proper date marked. Ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded. Informed PIC and provided additional information regarding process. New Violation.

Violation of Code: [511-6-1.07(6)(b)]
Observed a container of degreaser place on container near hand sink (unlabeled). Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Informed PIC. (COS) by removing and labeled. New Violation.

Violation of Code: [511-6-1.04(7)(c)]
Food items stored in coolers not labeled. Informed PIC. New Violation.

Violation of Code: [511-6-1.03(5)(i)]
Observed (1) employee without proper hair restraints. Observed another employee with flowing hair. Informed PIC (not hair restraint available for employee). Secord employee pull hair back into ponytail. New Violation.

Violation of Code: [511-6-1.02(1)(d)]
Previous inspection not posted. New Violation. Observed corner of door with heavy debris. Advised to pressure wash floor to remove buildup on floor and corners. Informed PIC. New Violation. Observed dead bees in front window of store, fruits flies and flies throughout store. Back door opened at time of inspection. Informed PIC to keep to closed. Employee has also prepping food (packaged mangos). New Violation.
