Billy’s Clubhouse, located at 1580 Forest Hill Rd, received a score of 67 points out of a possible 100 points during their 10/14/2021 inspection by the Georgia Department of Public Health.
The violations noted by the inspector were:
Violation of Code: [511-6-1.03(2)(m)]
Observed PIC ( Person-In-Charge) could not provide copy of employee health form agreement for female worker ( who stated was apart of the cleaning crew); however, observed this female worker preparing pizzas. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. EHS advised PIC, all employees must have forms kept on the premises at all. PIC had female worker to complete form while EHS was presence. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.04(1)]
Observed several boxes moldy, fuzzy rotten whole oranges, limes, and lemons stored inside the beer walk in cooler. Per speaking with PIC, staff are to routinely check fruits and discard if moldy or rotten.Condition. Food shall be safe, unadulterated, and honestly presented. EHS advised PIC, all food items must be in good condition at all times. PIC will have staff to go through box of lemons, oranges, and limes discard moldy, rotten fruit. Corrected On-Site. Violation of Code: [511-6-1.05(7)(a)1]
Observed black, brown sticky, residue inside the soda dispenser gun (1x) at the bar. Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
Equipment food-contact surfaces and utensils shall be clean to sight and touch. EHS advised PiC, all food contact surface and utensils must be cleaned to sight and touch. PIC will cleaned inside both soda dispenser gun. Corrected On-Site. Repeat Violation.
Violation of Code: [511-6-1.05(7)(a)1]
Observed pink, black mold accumulating on the interior surfaces of the ice machine storing ice. Ice is a food item. Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
Equipment food-contact surfaces and utensils shall be clean to sight and touch. EHS advised PIC, all food contact surface and utensils must be cleaned to sight and touch. PIC will thoroughly cleaned ice machine and discard ice. New Violation.
Violation of Code: [511-6-1.07(6)(d)]
Observed facility are using a pesticide spray bottle unlabeled and stated it was bleach per speaking with owner stored on shelving rack underneath the alcohol bottles.Only those poisonous or toxic materials that are required for the operation and maintenance of the foodservice establishment, such as for the cleaning and sanitizing of equipment and utensils, and the control of insects and rodents shall be allowed in a food service establishment. Pf The requirement in paragraph 1of this subsection does not apply to packaged poisonous or toxic materials that are for retail sale. PIC will purchase spray bottles and labeled accordingly. Observed a whole gallon of milk with expiration date of 10/5/2021 stored in the beer cooler as of 10/14/2021.Expired Foods. Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. PIC discard whole gallon of milk. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.04(4)(q)]
Observed facility are storing all alcohol bottles on shelving rack within the bar area without any form of coverings. Where it is not exposed to splash, dust, or other contamination. PIC will alcohol caps covering for bottles. New Violation.
Violation of Code: [511-6-1.03(5)(i)]
Upon entering the facility, observed female worker preparing pizzas without wearing any form of hair restraints. Hair Restraints.
Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles. EHS advised PIC, all employees handling and preparing food. EHS provided a hair restraint. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.02(1)(f)(1)]
Observed facility current Food Service Permit was not posted for public viewing. Per speaking with PIC, was unable to find Food Service Permit when EHS asked to view. Post the permit in a location in the foodservice establishment that is conspicuous to consumers. EHS advised PIC, all foodservice establishment current Food Service Permit must be posted for public viewing. PIC request another copy of Food Service Permit. Repeat Violation. Correct By: 10/14/2021
Violation of Code: [511-6-1.07(3)(d)]
Observed no hand washing signage posted at both bar hand washing sink. All hand washing sinks must have hand washing signage posted. Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. PIC will add hand washing signages. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.07(3)(d)]
Observed no hand washing signage posted at the hand washing sink in men’s restroom near slot machines. Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. PIC will purchase hand washing signage for men’s restroom. New Violation.
Violation of Code: [511-6-1.05(10)(a)]
Observed facility has several glassware of cups being stored on top of one another with visible wetness presence at the facility bar area. Protection of Clean Items.
Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils:
Shall be air-dried or used after adequate draining before contact with food; and May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry. EHS advised PIC, facility must allow utensils, glasses to air dry properly to prevent any form of contamination. PIC will speak with bartender in regards to properly storing glassware. New Violation.
Violation of Code: [511-6-1.05(10)(e)1&3]
Observed (3x) single-services pizza boxes sitting directly on the floor in the main kitchen.At least 6 inches (15 centimeters) above the floor. PIC elevate the pizza boxes. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.05(6)(r)]
Observed facility are using a single-service clear plastic cup as a scooper for the shredded cheese stored in the container. single-Service and Single-Use Articles, Use Limitation.
Single-service and single-use articles may not be reused. The bulk milk container dispensing tube shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head. PIC discard cup inside the container of shredded cheese. Corrected On-Site. New Violation.

Observed employee drinks ( 2x Dr. Pepper bottles and (1x) Mellow Yellow) sitting on top of kegs in the beer walk in cooler, and observed (2x) 2-go plates of Panda Express sitting on top of kegs in the beer walk in cooler.Designated Areas for Employee Activity.
Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. PIC will create desginated area for employees personal belongings. PIC removed all items. Corrected On-Site. New Violation.
