Church’s Chicken, located at 351 Riverside Pkwy, received a score of 67 points out of a possible 100 points during their 11/01/2021 inspection by the Georgia Department of Public Health.
The violations noted by the inspector were:
Violation of Code: [511-6-1.03(2)(m)]
Did not observe employee health policy in place. Was not able to verify food employees were informed of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food CA: Food employees and conditional employees are to be informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Reviewed and discussed reportable food borne illnesses and their report symptoms. Must be corrected within the next 2 days Repeat Violation.
Violation of Code: [511-6-1.06(2)(c)]
Observed hand sink water temperature at 87 F. Not able to maintain hot water temperature. CA: A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet. New Violation.
Violation of Code: [511-6-1.03(6)]
Did not observe vomit and fecal clean up procedure or supplies. CA: A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. Observed a can of jalopenos uncovered inside of Prep Top Cooler in Main, CA: Unless cooling, food shall be stored covered New Violation. Must be corrected within the next 2 days Repeat Violation.
Violation of Code: [511-6-1.05(7)(a)1]
Observed pink slime and mold on baffle inside of ice machine. Observed dishes stored as clean with grease debris. Observed black build up on white clear portion of drink nozzles at drink machine near drive thru CA: Food contact surfaces shall be clean to sight and touch. Repeat Violation.

CA: TCS foods being cold held shall be 41 F or below
COS: Operator stated raw chicken wash was prepared at 2 PM so wash placed on ice bath. Operator stated raw chicken breast and mixed pieces were taken out recently to batter and fry so placed inside of walk in cooler to cool. Corrected On-Site. Repeat Violation.

Did not observe thermometer inside of Prep Top Cooler in Main Kitchen.
CA: Thermometer measuring ambient air temperature shall be placed inside of warmest portion of coolers. Repeat Violation.

Observed boxes of single use plates and wrappers stored on floor in Main Kitchen.
CA: Single use utensils shall be stored at least 6 inches off the floor to prevent contamination. New Violation.
Violation of Code: [511-6-1.05(7)(a)2,3]
Observed doors and handles of Prep Top Cooler, oven, cooler storage unit, and walk in cooler heavily soiled with food and grease debris. New Violation.
Violation of Code: [511-6-1.07(5)(a),(b)]
Observed walls, floors, and ceiling tiles soiled with food splash and debris throughout entire Main Kitchen area.
CA: Physical facilities shall be maintained clean. New Violation.
Violation of Code: [511-6-1.07(2)(m)]
Observed back door propped open.
CA: Entrance of pests shall be prevented by the use solid, self-closing, tight-fitting doors New Violation.
