COS: PIC ensured raw shrimp stored above raw beef and raw chicken stored on bottom separate from all ready-to-eat items in reach-in cooler. Container of butter was discarded. PIC ensured chicken stored on bottom shelving in reach-in cooler separate from raw shell eggs and away from any ready-to eat items. PIC removed ribs from vegetables, vegetables were not contaminated by juices.

RCA: Except as specified in subsections (5)(a)3, and (5)(a)4(iv), and (9)(a)3 of this Rule, if an animal food such as beef, eggs, fish, lamb, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder, as specified in paragraphs 2 and 3 of this subsection using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Additional remarks:
Facility management is to appear for an informal hearing on Tuesday, February 3, 2026 at 10:00am. Informal hearing will be held at the Environmental Health Office located at 1395 Eisenhower Dr. (building at the back of the main health department).
Pork ribs, whole cuts of beef, raw chicken, raw fish, fish cakes, raw shell eggs, cooked pasta, milk, heavy cream, sour cream were all discarded.
