Malinalli Sabores Autóctonos, located at 6717 Hawkinsville Rd, received a score of 63 points out of a possible 100 points during their 10/17/2021 inspection by the Georgia Department of Public Health.
The violations noted by the inspector were:
Violation of Code: [511-6-1.03(1)(a)-(c)1,4,5,6-14,15, 16]
Observed PIC failing to demonstrate knowledge of food safety requirements and compliance with the Chapter due to the following: food from unapproved source, temperature violations, and maintenance of plans and documentation (i.e. employee health forms and invoices for fish). Violation of Code: [511-6-1.03(3)(c)]
Observed facility displaying a CFSM certificate that expired in 2020 in the service window of the facility. An expired CFSM shall not be displayed. Facility is to remove and obtain current CFSM certificate for an employee with managerial/supervisory duties. New Violation. Correct By: 12/17/2021
Violation of Code: [511-6-1.03(2)(m)]
Observed PIC failing to carry out assigned duties in terms of employee health reporting. Observed facility unable to provide any verifiable means that employees had been informed of the conditions in which they should report. All employees must be informed in a verifiable manner the conditions that must be reported. A DPH form will be provided which may be used for compliance. New Violation. Correct By: 10/17/2021

Observed no soap available at the only handsink in the facility. At all times, handsinks must be supplied with soap. PIC provided dish soap for EHS to wash hands while hand soap to be purchased and placed at sink. New Violation. Correct By: 10/17/2021
Violation of Code: [511-6-1.06(2)(o)]
Observed a green scrubbie and what appeared to be pieces of cilantro in the handsink when first attempting to wash hands. Handsinks may only be used for the purpose of handwashing and may not be used for any other purpose. Items removed to wash hands. PIC is to train all staff. Corrected On-Site. New Violation.

Observed facility lacking the supplies and written instructions to address fecal/vomitus contamination events. All facilities must have a plan in place to address such events if they occur. Facility is to purchase and/or assemble kit with needed items. New Violation. Correct By: 10/18/2021
Violation of Code: [511-6-1-.08(1)(k)]
Observed facility removing zip lock bags of product from the back of the pick-up truck while at the facility preparing to knock on the back door. When asked, PIC stated that products were sauces and that it had been prepared at home. Informed PIC that home prepared items were not allowed for service in the facility. PIC discarded. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.04(2)(d)]
Observed whole fish (that PIC identified as tilapia) stored in cooler with no means of identification or packaging. Furthermore, when asked, facility could not provide invoices or receipts for product. All foods must come from an approved source. PIC discarded. Corrected On-Site. New Violation.
Violation of Code: [511-6-1.04(6)(f)]
Observed multiple time/temperature control for safety foods (see foods and temperature below) in chest freezers with limited ice above the required 41F or less for cold holding. Per PIC, product had been in units overnight. All time/temperature control for safety foods intended for cold holding must maintain 41F or less. Out of temperature products discarded. Corrected On-Site. New Violation.

Observed OFF bug spray stored in the cabinet next to and with seasoning and sauces in the outdoor cabinetry. All medicines must be stored to prevent contamination of food, equipment, utensils, and/or single service items. PIC spoke to staff. All items to be rearranged. New Violation. Correct By: 10/17/2021

Observed Visine eye drops stored between various seasonings on the shelf in the trailer across from the maketable. Observed the need for additional cold holding equipment as facility was attempting to hold multiple TCS foods in 3 insulated coolers (non-commerical grade) using ice. All facilities shall have enough equipment in sufficient capacity to meet the storage needs of the facility. Encouraged PIC to purchase additional equipment. New Violation. Correct By: 10/19/2021

Observed the maketable/sandwhich prep cooler unable to maintain temperatures of 41F or less for cold holding. Though empty at time, unit is temping between 42-43F. Advised PIC not to store items intended for cold holding in unit until unit is able to maintain proper temperature. Contact maintenance for repair. New Violation. Correct By: 10/17/2021
Violation of Code: [511-6-1.07(5)(k)]
Observed evidence of pests through observations of live roaches on the wall near the hanging utensils and underneath shelf close to the handsink. All facilities must be free of pests and utilize effective measures to prevent the harborage of pests. PIC is to obtain pest control services. New Violation. Correct By: 10/18/2021
