Molinos Mexican Grill, located at 4852 Bill Gardner Pkwy, Locust Grove, GA 30248, received a score of 69 points out of a possible 100 points during their 08/18/2026 inspection by the Georgia Department of Public Health.

The violations noted by the inspector were:

Violation #1:
2-2B – proper eating, tasting, drinking, or tobacco use

Inspector Notes: Observed a box of pizza stored in the food preparation area. Also observed a consumed/open bottle of water stored next to margarine. An open can drink was stored next to food in the reach in freezer. An open bag of chips was also stored in the food preparation area. 511-6-1.03(5)(k)1&2 – Eating, Drinking, or Using Tobacco (C) (k) Hygienic Practices. 1. Employees shall not use any form of tobacco products, or electronic devices that simulate tobacco smoking, while engaged in food preparation or service, nor while in areas used for equipment or utensil washing and storage, food preparation or food storage. Employees shall only use tobacco products or electronic devices that simulate tobacco smoking in approved designated areas. 2. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed.

Inspector Notes: Observed a container of moldy raw chicken/milk mixture and a container of coleslaw stored in the reach in cooler. Inspector Notes: Observed expired foods including raw chicken nuggets (8/13), sliced pineapples (8/11), Poblano’s with peppers and eggs (8/15/), and coleslaw (8/16) were stored past the disposal date. 511-6-1.04(6)(h) – Ready-To-Eat Time/Temperature Control for Safety Food, Disposition (P) (h) Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition. 1. Inspector Notes: Observed peppers and onions stored next to the handsink in the kitchen area. Three containers of margarita mix were not stored six inches above the floor. Also observed a continuous leak coming from the AC unit leaking into the three compartment sink and over clean dishes stored in the three-compartment sink area. Also observed an employee using the handsink to fill water pitchers. PIC instructed to not use the handsink for refilling or filling water pitchers to prevent potential cross contamination from handwashing. 511-6-1.04(4)(q) – Food Storage (C) (q) Food Storage. 1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. 2. Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts. 3. Pressurized beverage containers, cased food in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture.

Inspector Notes: Observed the inspection score was posted behind a fixture. The inspection report and current food permit must be posted within 15 feet from the front door, five to seven feet from the floor, and within one food of reading distance. 511-6-1.02(1)(d) – Displaying of the Inspection Report (C) 1. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. 2. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information. 3. At food service establishments with no primary or public door, the current inspection report shall be prominently displayed at all times where the documents can be read by the public from a distance of one foot away. If requirements of this paragraph are not possible because of physical restrictions, a location will be determined as approved by the Health Authority. 4. The most current inspection report for mobile food service units and extended food service units issued by the local Health Authority having jurisdiction for its inspections shall be prominently displayed in public view during all hours of operation. Such inspection reports shall be prominently displayed at the point of service where the documents can be read by the public from a distance of one foot away. 5. A food service establishment inspection report addendum need not be displayed, but shall be made available by the food service establishment to the public upon request.

Inspector Notes: Observed a wire basket skimmer stored wedged between the wall and equipment. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container; 2.In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon; 3.On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a); 4.In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 5. Inspector Notes: Observed several leaks coming from the AC unit above the three compartment sink. PIC advised that a work order was placed to have the leak repaired. PIC will email a copy of the work order to robert.lewis1@dph.ga.gov and sabrina.young@dph.ga.gov by 8/19/26.

Inspector Notes: Observed live fruit flies in the bar area. Also observed a dead house fly in a bottle of liquor. PIC corrected on site by discarding the bottle of liquor. A dead cockroach was also observed in the employee restroom. 511-6-1.07(5)(k) – Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.

Additional remarks:
Inspection was conducted with Sabrina Young (EHS IV). Discussed shrimp disclosure, allergens, false inspections, and state audit.

Due to the unsatisfactory score, a reinspection will be conducted on 8/27/26. A reinspection fee of $100.00 must be paid to the Henry County Environmental Health Office no later than 8/20/2026. A risk control plan must be submitted to our office or emailed to robert.lewis1@dph.ga.gov and sabrina.young@dph.ga.gov by 8/20/2026
