Inspector Notes: Person-in charge not performing responsibilities as it pertains to their establishment by not ensuring: TCS foods are hot held at the proper temperature of 135F or greater, TCS foods are properly covered when stored in cooler units when not actively cooling, opened packages of food items are not adulterated, maintaining cleaning throughout facility, removing dead pest, cleaning of the ventilation system to prevent accumulation of grease, proper thawing methods, and preventing contamination of food by storing off the floor and in clean cooler unit. COS, TCS foods were covered, dead pest were swept up, adulterated package of diced garlic discarded, and improper thawing of TCS foods were corrected by placing in cooler and running water by PIC upon request. All other violations must be corrected within timely manner and notify when corrected.

Inspector Notes: Observed rodent activity as evidenced by rodent droppings found & Victory traps at prep table and one on floor near hand wash sink.

Additional remarks:
*Facility was asked to voluntarily close down due to imminent health hazard, gross unsanitary occurrences/condition.

When an establishment receives a grade of “U”:

1) A follow-up inspection will be conducted within 10 days.

2) An additional routine inspection shall be conducted within 12 months in addition to the regular routine inspection.

*All violations for priority items ( within 72 hours) and priority foundation items ( within 10 days) that are not corrected on site shall be corrected in a timely manner as required by Chapter 511-6-1.
