On Tuesday January 13, 2026 at approximately 1107 hours, I Officer Jinright, with the Chatsworth Police Department Patrol Division, was dispatched to 502 G.I. Maddox Parkway (Food City) Chatsworth, Murray County, Georgia in reference to a motor vehicle accident.

While en route Murray Dispatch advised that the at fault vehicle had left the scene. The vehicle was described as a black or silver truck with damage all over and no rear bumper. When I arrived on scene [VICTIM #1] said that a white male driving a silver truck had struck her, pulled over to the Food City gas station, got out of his vehicle, and looked at the damage. She said the male then got back into his vehicle and left the scene. [VICTIM #2] who was in the vehicle with her 3 juvenile children gave the same account of the incident. I got all information needed for the report and looked over the area for the vehicle but could not find it.

I went inside Food City and asked if I could watch the surveillance footage from the gas station to see if I could identify the vehicle and driver. The footage showed the suspect vehicle, a silver Toyota Tacoma, traveling west on G.I Maddox Parkway make a left hand turn into the parking lot. The suspect vehicle struck [VICTIM #1]’s vehicle so hard it made her vehicle roll backwards.

The footage showed the vehicle pull onto the lot of the gas station where a white male wearing a blue shirt, blue jeans, and boots that were covered in white dust. The male got back into the vehicle and left, making no attempt to check on [VICTIM #1] or exchanging information.

Seeing the white dust on the boots of the male made me believe that the male worked at one of the local talc mills. When I left Food City, I responded to Cimbar on Old Federal Road North. I located the suspect vehicle in the employee parking lot. I spoke to one of the employees who told me Chad Lee Wade was the driver of the truck. I asked him if he could go get Chad so I could speak with him and he did.

Chad emerged from one of the warehouses and I called him over. I asked Chad, who at first denied even leaving his job site, why he left the scene. Chad said that he did not know. I gathered all of Chad’s information. He gave me an insurance card for [REDACTED BY AGENCY] and his driver’s license. I told Chad that it was an ongoing investigation and he could potentially be receiving charges for leaving the scene of an accident.

I was able to find the correct code section and issued a warrant for Chad for Hit and Run.
