New Orient Express, located at 425 Sigman Rd NW # 122, Conyers, GA 30012, received a score of 60 points out of a possible 100 points during their 09/23/2026 inspection by the Georgia Department of Public Health.
The violations noted by the inspector were:
Violation #1:
1-2A: .03(2)(a)-(n),(p)PIC does not have active managerial control. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)
Violation #2:
2-2D: .06(2)(o)Observed a stainless steal sponge with food debris on is stored in the only handsink in the kitchen. A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer’s instructions. (Pf)
Corrective Actions: Sponge was removed to an approved location by food employee.
Violation #3:
3-1C: .04(1)Observed a roach stored in a large container of uncooked rice. Food shall be safe, unadulterated, and honestly presented. (P)
Corrective Actions: PIC (person in charge) removed the roach with his bare hands to discard. Rice was discarded.
Violation #4:
4-2B: .05(7)(a)1Observed the following with food debris; 1. drink nozzles in the server area 2. interior microwave 3. interior of ice machine 4. clean knives 5. can opener Observed the interior bottom of the plastic container storing egg rolls with mold on them. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)
Corrective Actions: PIC had the can opener, drink nozzles, microwave, and knives cleaned. ***SECOND REPEAT OF CODE VIOLATION. OBSERVATION ON NEXT ROUTINE INSPECTION WILL REUSLT IN PERMIT SUSPENSION.***
Violation #5:
6-1A: .04(6)(f)Observed two containers of noodles, and a container of egg rolls cold holding above 41F in the walk-in-cooler. Observed multiple containers of egg rolls and two containers of cooked chicken cold holding above 41F in the reach-in-cooler. Reach-in-cooler held ambient air of 43.7F. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below. (P)
Corrective Actions: PIC discarded the noodles and egg rolls. PIC removed the containers of egg rolls and cooked chicken to the walk-in-cooler.
Violation #6:
12A: .04(4)(i)Observed a card board box, containing food items, with mold on it. Observed card board boxes containing food, with ice on them in the walk-in-freezer. Packaged food may not be stored in direct contact with ice or water if the food is subject to the entry of water because of the nature of its packaging, wrapping, or container or positioning in the ice or water. (C)
Corrective Actions: PIC had food employees discard the cardboard boxes.
Violation #7:
15A: .05(1)(a)Observed a container with egg rolls stored on menus. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions, and shall be: (P) safe (P), durable, corrosion-resistant, and nonabsorbent. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. (C)
Corrective Actions: Menus and egg rolls were discarded.
Violation #8:
15A: .05(6)(a)Observed the reach-in-freezer held ambient air of 43.7F. Observed the walk-in-freezer door in disrepair. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections .05(1) and .05(2) . (C)
Violation #9:
15C: .05(7)(a)2,3Observed the walk-in-cooler shelving held heavy food debris. Observed the nonfood contact tops held heavy food debris that appeared to be molded. Observed the exterior container with egg rolls containing debris. Observed an accumulation of ice on the walk-in-freezer pipping, by the walk-in-freezer entrance door, and in the reach-in-freezer. Observed the shelving in the storage area with heavy food debris. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (C)
Corrective Actions: PIC cleaned the non food contact tops and container that stored egg rolls.
Violation #10:
17C: .07(2)(e)Observed wood in the walk-in-cooler being used as a non food contact surface. Mats and duckboards shall be designed to be removable and easily cleanable. (C)
Violation #11:
17C: .07(2)(a)Observed the flooring, the wall, and ceiling in the storage area does not have approve finishes. Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. (C)
Violation #12:
17C: .07(5)(a),(b)1,2,3Observed the walk-in-cooler flooring by the fan with food debris. Observed the wall in the storage area by the reach-in-freezer with heavy debris. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)
Violation #13:
18: .07(5)(k)At the beginning of the inspection, observed two dead roaches floating in standing water in a soy sauce bucket. The soy sauce bucket was stored in the vegetable sink. Food employee then emptied the bucket in the vegetable sink. One of the dead roaches was still observed in the vegetable sink. Observed a dead roach stored in the container of uncooked rice. Observed two dead roached floating in a container storing cleaning chemicals. Observed three alive roaches in the storage area by the reach-in-freezer. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; (C) 2. Routinely inspecting the premises for evidence of pests; (C) 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under Rule .07(6)(e),(m),(n); (Pf) and 4. Eliminating harborage conditions.(C)
Corrective Actions: Food employee emptied the soy sauce bucket in the vegetable sink. Food employee clean the vegetable sink. PIC removed the dead roach with his bear hands from the uncooked rice. Rice was discarded. Container of cleaning chemicals was dumped in the grease trap along with the dead roached.
Violation #14:
18: .07(2)(m)Observed the back exit screen door does not self close and has a hole at the bottom. Observed the front entrance door does not self close. Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; (C) 2) closed, tight-fitting windows; (C) and 3) solid self-closing, tight-fitting doors. (C)

Hot-Holding was assessed and incompliance unless note in the temperature log.
Ensure flooring in the walk-in-freezer is in good repair.
Gave a list of approve finish to PIC.
A follow up will be conducted by 10/2/26
A required routine inspection will be conducted 60 days after follow up.
Suggested that PIC train his wife and food service cook on food safety and cleaning.
Fees will be charged accordingly for any violation which cannot be corrected on-site during inspection.
Repeat violations may result in permit suspension.
Corrections of violations during and after inspection does not change the inspection score.
