Violation Of Family Violence Order

On 05/17/2026 I was dispatched to [REDACTED BY AGENCY] in regards to a domestic that had already taken place.

Upon my arrival I met with the victim, [VICTIM #1]. [VICTIM #1]’s brother, [WITNESS #1], was also on scene after she contacted him via her computer. [VICTIM #1] stated she was asleep when her husband, Jack Culver, entered the apartment using a spare key. [VICTIM #1] stated that Jack began arguing with her about her speaking to other men on her phone. [VICTIM #1] told me that she and Jack are separated currently, but he was still mad at her for speaking to other men.

[VICTIM #1] stated that during the argument, Jack took her phone and slammed it against a counter and broke the phone. [VICTIM #1] stated that Jack then left the apartment and [VICTIM #1] was able to contact [WITNESS #1] via her computer. [VICTIM #1] showed me her phone and it was very damaged. [VICTIM #1] stated that the phone no longer worked. [VICTIM #1] informed me that Jack has a TPO or bond conditions that state he is not allowed to contact her or come to her residence. [VICTIM #1]’s mother still had his location on her phone and it showed that he was in an apartment on Stone St. I then relocated to Stone St to attempt to speak to Jack. I was told that Jack drove a black Nissan Titan.

Upon my arrival at Stone St, I did not see any black Nissan Titan. I was then told by dispatch that [WITNESS #1] had called back saying Jack returned to [REDACTED BY AGENCY] and attempted to enter the apartment again, but left the scene shortly after. I then returned to [REDACTED BY AGENCY].

I met with [WITNESS #1] who stated that Jack returned shortly after I had left to find him. [WITNESS #1] showed me a video that showed Jack attempting to enter the apartment but [WITNESS #1] and [VICTIM #1]’s mother were blocking him from entering. [WITNESS #1] stated that Jack then left. I was able to see that Jack’s location again and he was driving south on I-75. [VICTIM #1] was able to provide me with Jack’s phone number and I had dispatch link me up with him on a phone call.

While speaking to Jack over the phone, I asked Jack what had taken place. Jack stated that he was upset about [VICTIM #1] cheating on him. He stated that the two were married but are in the process of getting divorced. Jack stated that hе argued with [VICTIM #1], but denied knowing anything about her phone being broken. Jack then asked if he had a warrant for his arrest and mentioned having a TPO order. I told Jack I was in the process of obtaining his bond conditions and would have to confirm if they were still in place. Jack then stated he did not want to speak further and our conversation ended.

Jack then called back later and asked again about what the conclusion of this incident would be. I told Jack that if there were active bond conditions or a TPO that he violated, then a warrant would be taken out for his arrest. Jack stated he understood and he would be willing to turn himself in on Monday, but he would need to speak to his employer first so he could keep his job.

I was able to obtain a copy of Jack’s bond conditions from a previous arrest in December of 2025 in which he was arrested for family violence involving [VICTIM #1]. The bond conditions stated that Jack is not allowed to have contact with [VICTIM #1] or be around her. The copy of the bond conditions will be attached to this report. The images of the damaged phone and the video of Jack will also be attached to this report. A crime victim’s bill of rights was left with [VICTIM #1] as well.

Due to there being active bond conditions, I will be seeking a warrant on Jack for 16-5-95 violation of criminal family violence order and 16-5-90 for stalking. I will update this report when the warrant has been submitted. I will not be seeking charges for [VICTIM #1]’s phone being broken due to Jack stating he did not break the phone and there not being any evidence showing he did.
