Additional remarks:
During routine inspection found facility that had been previously permitted as Aunt M’s, now operating as K’s Kitchen (operating without a valid permit). This inspection is for a provisional permit (expires in 60 days). The following will be needed before a permanent permit can be issued, unless you show proof you have been permitted through the Department of Agriculture:
1. Submission of a Food Service application under the new name/ownership, along with residency verification form
2. Transfer of ownership fee paid ($100)
3. Copy of Breading Procedure
4. Documentation of employed CFSM or employee enrolled in CFSM class (cannot be CFSM for Subway, since that is separate food service operation)
