[NARRATIVE #1 on June 28, 2026]

On June 28th, 2026, at approximately 18:31 hrs, I was dispatched to Walmart at 600 Carrollton Villa Rica Hwy in reference to someone drinking alcohol while driving.

The complainant described a black Ford F-150 with a camper top, GA license plate [REDACTED BY AGENCY], parked in front of Pet Sense. A white male wearing a green tank top, shorts, flip-flops, and a hat was seen drinking a beer in his truck and then walking into Walmart. Upon arrival, I saw the vehicle matching the description pull out of its parking spot, and I saw the driver take a drink of something.

I activated my blue lights, which triggered my dashcam, and I conducted a traffic stop in the parking lot, pulling the F-150 (GA-[REDACTED BY AGENCY]) over. I activated my body-worn camera and approached the vehicle. I made contact with the driver, and I peered into the vehicle, seeing no drinks anywhere. I asked him where the drink he just had was, and he pointed to the back seat. I had him give me the drink, and he pulled out a lime Michelob Ultra beer bottle that was opened and not full. I obtained his driver’s license and identified him as Matthew Slowik. I asked him how much he had to drink, and he said two Michelob Ultra beers. I had him step outside of his vehicle and in front of my patrol car. I asked if he would perform an evaluation for his intoxication, and he said that he would. I then requested Cpl Ellinas to be en-route to perform FSFT.

Cpl Ellinas arrived and performed the evaluation on Matthew. I searched the vehicle for more open containers anc found no more. It is noted that there was a full case of beer purchased from Walmart in the front passenger seat.

Cpl Ellinas finished the evaluation and advised me that Matthew was under the influence of alcohol. I put Matthew in handcuffs and placed him under arrest for DUI- Less Safe (OCGA: 40-6-391(A)(1)) and Open Container (OCGA: 40-6-253). I ensured his handcuffs were double-locked and that the backseat of my patrol vehicle (0594) was free of contraband. I read the GA Implied Consent Notice for Suspects Age 21 or Over card to Matthew, and he consented to blood. I transported him to the Villa Rica PD, booked him in, and fingerprinted him.

Ofc Roseberry stood by with the vehicle, which was later towed by Huey’s Wrecker Service for impound.

[REDACTED] with Blueline Forensics, Inc arrived at the PD and drew two vials of Matthew’s blood. I submitted the vials into evidence for testing (VR-019338).

I read Matthew his citations and explained his court date. I put Matthew in handcuffs and ensured they were double-locked, and fastened him by seatbelt in the back of my patrol vehicle. I transported him to Carroll Co SO, where he was turned over for housing.

[NARRATIVE #2 on June 28, 2026]

On June 28, 2026, at approximately 1849 hours, I, Corporal Ellinas responded to Walmart (600 Carrollton Villa Rica Hwy) as backup to Officer Holder. See Officer Holder supplement.

I spoke with Matthew Slowik. While speaking with him, I saw that his eyes appeared to be bloodshot and watery. When asked, he advised that he had consumed two “Michelob Ultra Lime Cactus” and further advised that he “had one at home and one on the way” to Walmart. I asked Slowik if he would participate in Standardized Field Sobriety Testing to ensure he was able to operate a motor vehicle on the road and he advised he would. While speaking with Slowik I saw that his eyes were bloodshot and watery.

Standardized Field Sobriety Testing (SFST)

Horizontal Gaze Nystagmus (HGN): 6 of 6 clues

Slowik was positioned in front of a Villa Rica patrol vehicle, on relatively flat and level pavement. Slowik was directed to stand with his feet together and his hands by his sides. I asked Slowik if he wore glasses or contacts, he advised he wore glasses but only for driving at night. I asked Slowik if he had any recent head injuries, he advised he did not. I asked Slowik if he had any issues with his eyes and he advised he did not. Slowik was instructed to follow a stimulus, an ink pen, with his eyes while keeping his head stationary. Slowik advised he understood the instructions and indicated he was ready to begin the assessment. Slowik had equal tracking and equal pupil size in both eyes.

Slowik displayed a lack of smooth pursuit in his left eye.
Slowik displayed a lack of smooth pursuit in his right eye.
Slowik displayed distinct and sustained nystagmus at maximum deviation in his left eye.
Slowik displayed distinct and sustained nystagmus at maximum deviation in his right eye.
Slowik displayed onset of nystagmus prior to 45 degrees in his left eye.
Slowik displayed onset of nystagmus prior to 45 degrees in his right eye.

Walk-and-Turn (WAT): 2 of 8 clues

I administered the walk-and-turn evaluation to Slowik. I asked Slowik if he felt the surface was flat and level. Slowik advised it was. I explained the instructions and demonstrated the evaluation. I asked Slowik if he had any problems with his legs or feet that would prevent him from walking in a straight line and he advised he did not.

Slowik missed heel to toe while attempting to walk in a straight line.
Slowik used his hands to balance while walking
Slowik did not execute a proper turn as was demonstrated

One-Leg Stand (OLS): 1 of 4 clues

I administered the One-Leg Stand evaluation to Slowik. I explained the instructions and demonstrated the evaluation. I asked Slowik if she understood my instructions. Slowik advised she did. I asked Slowik if he had any problems with his legs or feet that would prevent him from walking in a straight line and he advised he did not. Slowik used his hands to balance while attempting to stand on one leg.

This concluded the Standardized Field Sobriety Testing. I retrieved a department-issued portable alco-sensor and explained the instructions on how to provide a breath sample. I was able to collect a sample which read 0.107 BAC indicating a positive presence for alcohol
I informed Officer Holder of my findings.
