Sushi Misong in Duluth receives a 57% on their recent health inspection

Sushi Misong, located at 2300 Pleasant Hill Road in Duluth, received a score of 57 points out of a possible 100 points during their 04/16/2024 inspection by the Georgia Department of Public Health.

The violations noted by the inspector were:

Violation #1
1-2A: .03(2)(a)-(l), (n)Person in charge not demonstrating active managerial control (improper cold holding, adulterated food, improper cooling). Multiple public health interventions were made during inspection. It is the responsibility of the person in charge to ensure compliance with the food code by demonstrating active managerial control within their facility. (Pf)

Violation #2
2-2B: .03(5)(j)1&2At time of inspection, observed multiple employee drinks in unapproved cups stored above prep line cooler, next to customer food, and above prep tables. Observed a food handling employee chewing gum. Employees shall consume food and drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas (unless drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles). Corrective Actions: Employee relocated to the dining room. Employee gum discarded.

Violation #3
3-1C: .04(1)Observed multiple live fruit flies crawling in a large container ready to eat radish noodles. Food shall be safe, unadulterated, and honestly presented. (P)
Corrective Actions: Radish noodles discarded.

Violation #4
3-1D: .04(5)(f)Facility purchases raw sweet shrimp and raw scallops from Mega Mart and food is offered undercooked. Facility does not have a parasite destruction letter from the company who destroys the parasites. Facility has parasite destruction letter from Mega Mart. Sweet shrimp and scallops must be served fully cooked until the parasite destruction letter from the company who performs the destruction of parasites is provided and approved by the health authority. If the fish are frozen by a supplier, a written letter of parasite destruction from the supplier stipulating that the fish supplied are frozen to a temperature and for a time specified under subsection (5)(e) of this Rule may substitute for the records specified under paragraph 1 of this subsection. Violation #5
4-2B: .05(7)(a)1Observed black and pink mold like substance in the interior of the ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf)

Violation #6
6-1A: .04(6)(f)Multiple time/temperature control for safety (TCS) foods cold holding greater than 41F for more than 4 hours (see temperature log). Time/temperature control for safety food shall be maintained at 41ยฐF (5ยฐC) or below. (P)
Corrective Actions: Foods discarded.

Violation #7
6-1C: .04(6)(d)Multiple TCS foods did not cool to 41F in 4 hours or 6 hours (see temperature log). Time/temperature control for safety foods prepared from room temperatures or refrigeration temperatures shall be cooled from 70ยฐF to 41ยฐF (5ยฐC) or below within 4 hours. (P)
Corrective Actions: Foods discarded.

Violation #8
11A: .04(6)(e)Facility using improper Methods for cooling TCs foods. Foods cooling in cold hold units that are not working properly. Foods cooling while tightly wrapped and stacked. Foods shall be cooled using one or more methods approved by the health authority (i.e., small batches, thinner portions, ice wands, blast chillers, ice as an ingredient, placing in the walk-in cooler or walk-in freezer uncovered (or partially uncovered) as long as the food is protected from overhead contamination). Corrective Actions: Foods did not cool properly and were discarded.

Violation #9
12A: .04(4)(r)At time of inspection, observed lots of food stored outside (raw chicken, oil, produce). Per person in charge, food had been delivered about 10-15 minutes ago (did not observe employees brining foods into kitchen at time of inspection). Observed bags of rice and boxes of bottles alcohol stored in an outside storage unit with unapproved finishes. Food may not be stored in the following areas: 1) locker, toilet, or dressing rooms; (C) 2) garbage or mechanical rooms; (C) 3) under sewer linens that are not shielded to prevent potential drips; (C) 4) under leaking water lines, automatic sprinkler heads or lines which water has condensed; (C) 5) under open stairwells; (C) or 6) under other sources of contamination. (C) **2nd CONSECUTIVE REPEAT VIOLATIONS ON A ROUTINE INSPECTION**
Corrective Actions: Raw chicken was at a temperature of 41F or less and was moved into the walk in cooler. All foods from delivery and bags of rice were moved into the kitchen.

Violation #10
12D: .04(4)(g)Observed frozen raw shrimp thawing on the vegetable sink drain board. Raw fruits and vegetables shall be thoroughly washed in water, in a sink designated for that purpose only, to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in ready-to-eat form. (Pf)
Corrective Actions: Raw shrimp was placed in the walk in cooler and the vegetable sink was washed, rinsed, and sanitized.

Violation #11
14C: .05(10)(e)1&3Observed boxes of single use items stored on the floor in the outside storage unit. Discussed with manager that once the seal to the box has been opened, single use items must be moved to the kitchen since outside storage unit does not have approved, floors, walls, and ceilings. Single-service and single-use articles shall be stored in a clean, dry location, where they are not exposed to splash, dust or other contamination, and at least 6 inches above the floor and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. (C)

Violation #12
15A: .05(6)(a)The ambient air temperature of the walk in cooler and display prep cooler are greater than 41F. Observed lots of ice on the condenser in the walk in cooler. Observed lots of ice on the floor in the walk in freezer. Equipment must be maintained in good repair and proper adjustment.

Violation #13
15C: .05(7)(a)2,3Observed food debris on several pieces of equipment in facility (exterior surfaces of food containers, fryer, grills, walk in cooler fans). Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (C)

Violation #14
16B: .06(2)(r)The hot water nozzle at the handwashing sink in the kitchen is stripped. System must be maintained in good repair.

Violation #15
17B: .06(5)(r),(s)Observed lots of trash stored outside the back door of facility. Observed lots of trash between and behind outdoor storage units). Refuse, recyclables, and returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents. (C) **2nd CONSECUTIVE REPEAT VIOLATION ON A ROUTINE INSPECTION**

Violation #16
17C: .07(5)(a),(b)1,2,3Observed lots of food debris on the floors and walls throughout the kitchen and dish area. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)

Violation #17
17D: .07(4)(b)Observed 2 employee coats, cell phone charger, and speaker stored with and above customer food and items. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. (C)
Corrective Actions: Employee items relocated.

NOTE: All cold holding and hot holding temperatures in compliance, unless noted.
NOTE: Facility has adequate refrigeration when the door to the walk in freezer is left open (walk in freezer inside of walk in cooler). Food service permit will be SUSPENDED if the walk in freezer door is not left open.
NOTE: Invoices for live flounder provided.
NOTE: Ensure TCS foods are thawed under refrigeration.
NOTE: Ensure all wood and cement blocks are removed from facility.
NOTE: A follow up inspection will occur on 4/26/24.
NOTE: A required additional routine inspection will occur within the next 12 months.
3 consecutive REPEAT violations on a routine inspection may result in food service permit SUSPENSION.
