At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken.

Inspector Notes: Observed no employee health policy in place. The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food.

Ice bath was not adequate and pans were merely sitting on top of ice as opposed to sitting in ice. TCS foods must be cold held at 41ยฐF or below. COS by moving to another unit.

Inspector Notes: Observed substantial mold build-up on food storage shelves in walk-in cooler. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Shelves must be thoroughly cleaned and sanitized.

Additional remarks:
*Owner stated that facility will be closing next week, however this has been stated on various occasions over the past 2 years. If facility is planning on closing, proper food safety must still be followed while in operation. Potential closing is not an excuse for blatant neglect of facility or food handling. There were 8 repeat violations on this inspection. Follow-up inspection will be conducted within 10 days. Failure to receive a satisfactory score will result in facility being asked to close.
