On 06-06-2026, I was dispatched to [REDACTED] in reference to an Active Domestic.

Upon arrival, I made contact with the complainant/victim identified as [VICTIM #1] and she advised that her husband, Levi Maddox had accused her of being a child molester and saying her breath “smelled like di*k.” [VICTIM #1] stated that an argument then ensued between her and Maddox. She stated that she then went on to the back porch to get away from him. [VICTIM #1] stated he pushed her in the face several times and pushed her off the back porch, resulting in bloody scrapes on her left elbow, left lower leg and right hand. Her injuries were photographed and will be attached to this report.

We then made contact with Maddox in the backyard, I noticed while speaking with him that he had rips/tears on his shirt as well as blood. When questioned about what had occurred, he stated that nothing physical happened and looked back towards his mother in law, [WITNESS #1], who was sitting on the back porch listening and asked her to confirm his story, and that he did not physically attack [VICTIM #1]. Mrs [WITNESS #1] stated that he did actually put his hands on her and contradicted his version of events. Maddox attempted to walk away at this time and was taken into custody and placed in the rear of my patrol car.

I then returned to the back porch to question Mr [WITNESS #2] & Mrs [WITNESS #1] about what had happened and they advised that Maddox did push [VICTIM #1] off the porch and the 13 and 14 year old children witnessed the altercation from inside the sliding glass door. We had [VICTIM #1] and Mr [WITNESS #2] & Mrs [WITNESS #1] make written statements about what had occurred, which are attached to this report.

I also noticed a necklace on the ground near the back porch while getting statements and [VICTIM #1] stated it belonged to Maddox, corroborating that a struggle had occurred on the back porch. There is also a camera overlooking the back porch which should have captured the event but they were unable to show us any footage at the time of making this report.

While running Maddox through NCIC, I was advised that he had a Valid Warrant out of Covington GA. Maddox was then transported to the Byron Police Department where he was processed and booked for 1 count of Battery Family Violence, and 2 counts of Cruelty to Children – 3rd Degree. He was then transported to Peach County Jail for his charges and the arrest warrant and turned over to Deputies.
